Oil based drilling mud was spilled on containment (approximately 1 barrel). Spill was contained and removed. Upon its removal a hole in the containment liner was observed. It was determined that approximately five gallons of mud made it to the pad. At time of the inspection ES&G Environmental and Moody and Associates were doing an analysis of the spill area following Act 2 parameters. All contaminated materials were in sealed roll of boxes.

2003118 2011-09-02 No Violations Noted At the time of inspection, well sign, ES plan, and ESCGP-1 present. Access road adequately rocked and stable. Site gated with 24 hr security onsite. Well drilled, not yet in production. BMPs installed and functioning properly. No accelerated erosion or sedimentation observed. Site has been temporarily seeded and mulched providing temporary stabilization. Five frac tanks located onsite, noted that one tank was left with a hose attached (Chevron Rep Nick Staffieri contacted). Observed that area of previously reported spill has been cleaned up, yet disturbed areas have not yet been restored as operator is awaiting final analysis results to confirm all contaminates have been removed. A follow up inspection will be conducted to ensure that previously impacted areas have been successful remediated.
1972639 2011-05-13 No Violations Noted At the time of inspection, well sign (permit #), ES plan, and ESCGP-1 located onsite. Well spud 05/11/11. Access road stable. Rig located onsite actively drilling. Site secured by fence and 24hr security. Observed temporary (lined) containment pad located beneath rig in order to prevent/contain a possible pollution event. Temporary BMP﾿s installed according to ES plan. All culverts, outlets, and sediment traps appear adequately rocked. No accelerated erosion/sedimentation or IW issues are noted. Operator should continue to routinely monitor/maintain all BMP﾿S until final site restoration is achieved. All previously disturbed areas have been stabilized with vegetative blankets, with some growth observed. Soil pile has been temporarily seeded, minimal growth observed, although compost sox installed down gradient. No violations noted during this inspection.

Conductor pipe set at 69 ft using 20﾿ pipe. First stringer set at 679 ft with 13 3/8﾿ pipe using Gas block of 485 sacks with 15 bbl return. Second stringer was set at 1846 feet using 9 5/8﾿ pipe with 585 sks and 15 bbl return. Current depth was at 2,300 feet. Pilot hole set for 9000 ft. E&S controls were in place.

Well granted inactive status on 6/8/2012. Drilled, not yet frac'd. Well tag present. Tank on containment. Silt socks were removed at request of landowner. Site appears temporarily stabilized and no E&S issues observed although site snow-covered. Access road in good condition. Prior agreement with Department to begin restoration activities in spring 2013. At that time, Operator will remove 3 sed traps, soil pile will be used to restore cut slope, two collection ditches will remain, and gravel pile next to cut slope will be spread out. Department rep Samantha Foulk field-approved change to E&S plan. Upon completion of driling operations, topsoil will not be spread over the pad, the pad will remain rocked, and the pad size will not change.

Pad is not vegetated and remains stone-covered. Production equipment and brine tank surrounded by gate. Tank is on lined secondary containment. Tank appears sound. Two stockpiles of stone on site. Well spudded on 5/11/13, granted inactive status on 6/08/12. Dept. spoke to two reps from Chevron who arrived on site during inspection. Discussed well history and plans for restoration work. No E&S issues observed and access road is in good condition.
