2771853 Primary Facility 2018-09-05 Routine/Partial Inspection No Violations Noted This is a well that the operator plans to plug soon. Depending on how the #16 well goes, this may be plugged first. There was a pump jack hooked up to electricity onsite. The plugging operator was unsure if and how the power would be killed so he can move the jack out of the way. The well is in an open area of a pasture so it should be fairly easy to plug.
2778518 Primary Facility 2018-09-19 Routine/Partial Inspection No Violations Noted At the time of inspection, I had met with Larry Maul to discuss where he would set cement plugs. Rods and tubing were pulled from the well. TD had not been tagged yet. He hopes to be cementing by Saturday.
2780833 Primary Facility 2018-09-25 Routine/Partial Inspection No Violations Noted At the time of inspection, no activity was taking place. There were three 100 bbl plastic tanks onsite, as well as an open top tank for residual waste. The bottom of the Knox 5th is at 1060'. The TD was tagged at 1251'. Larry Maul had tubing down to 1164' to start plugging. Plan was to have cement 50' below the Knox 5th and up to 50' above the 1st Venango (which would be to 680'). There would be gel above this. A second cement plug would be set from 614' up to 464' (into the surface casing).
2792412 Primary Facility 2018-10-15 Routine/Partial Inspection No Violations Noted At the time of inspection, rig has been moved off the site. There was pea gravel added to near surface and a wooden plug was placed on top. This was being used to prevent surface water from entering the wellbore until the operator has the casing cut below grade. Well will be underground with a monument placed on it. The monument was onsite and consists of a steel plate with the permit number welded on it. Rods and tubing remain onsite. A mound of pea gravel is at the site as well.
2811609 Primary Facility 2018-12-04 Routine/Partial Inspection No Violations Noted Well is plugged. A swedge was placed with a steel pipe over the well as the monument. A tag has not been placed yet, but the tag is with the plugging operator at another well on the lease. It will be tagged by the end of the day.
2818355 Primary Facility 2018-12-21 Plugging(Includes Plugged/Mined Through) No Violations Noted This site located east off Foxview Road in a forested area, the third and last Vista well on an extensive access road. This well is plugged, all production equipment has been removed.
