2740373 Primary Facility 2018-06-08 Routine/Complete Inspection No Violations Noted A routine inspection of this deep vertical well was conducted June 8, 2018. The purpose of this inspection was to evaluate the well and well location in preparation of a potential upcoming plugging project. The approximately half-mile access road is off of Bushkirk road. At the time of inspection, the access road was able to be traveled by pickup truck and was in generally good condition with the exception of a few very wet/muddy areas. The approximately 80' x 100' location was mostly open with high grass/brush and few very small trees. The well is situated in the middle of the location. There was a wooden cellar, open but overgrown. Production line is off of wing valve below ground level in the cellar, spool and master valve on top of wellhead above ground level. There is above ground piping onsite to an old meter house and what appeared to be a blowdown line in the grass lined out to a small barrel.
2854179 Primary Facility 2019-03-12 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this deep, vertical (Oriskany) well was conducted March 12, 2019. The well is scheduled to be plugged under current state plugging contract OOGM 18-3. The site and access road have been cleared and stone has been spread on access road and location. Meter house and other production equipment have been isolated from the well and removed. At the time of inspection, the well is shut in at master valve. An analog pressure gauge atop master valve is isolated from the well bore. There was one annular vent observed at surface; visible venting was observed. No downhole work has been conducted on the well at this time. No obvious major issues were observed during this inspection.
