1885778 Primary Facility 2010-05-19 Routine/Complete Inspection No Violations Noted According to records, well completed on 11/24/03 to a depth of 3,791 ft. Active permit since 10/2/03. WRAC submitted and approved on 12/5/03. Well head completed with 8¿ pipe reduced to 4¿ and reduced to 2¿ production line with 53shut off valves. Production equipment has been removed from site and well is currently not producing. According to Chad Wagner, the well was shut in during the fall of 2009. No well tag was present. No IW issues during inspection surrounding the site. Slight E&S issues at the site. Site has needs additional restoration activity. Took pictures of the site. Request WQ personnel perform inspection of the site. Talked to Chad Wagner (Dorso) field supervisor about issues and Dorso is planning to plug well this fall of 2010.
