2815651 Primary Facility 2018-12-14 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Seneca Resources E16-PHC-C well pad on December 14, 2018. There is one well located on the well pad. This well is currently in "regulatory inactive status". A ~1" black hose is connected to the well head and is discharging fluids via the black hose into a tote stationed in secondary containment. No other production equipment is stationed onsite.

The well pad has been reduced in size. All restored areas are well vegetated. The Department has approved a notice of termination for the ESCGP and has approved the well site restoration report.

No violations noted.

2846956 Primary Facility 2019-03-01 Routine/Complete Inspection No Violations Noted A routine (pre-plugging) inspection of this vertical, uncompleted, unconventional well was conducted March 1, 2019. Plants and Goodwin rig 12 and equipment are staged on location for final plug and abandonment. Plugging was scheduled for today but operations have been delayed. At the time of inspection, the location was snow covered. Both 4 1/2" and 8 5/8" casings are shut in. No obvious issues were observed at this time.
2848387 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted The weather is 33 Deg. F and sunny at time of inspection. Routine inspection of Seneca Resources Corp., PHC Pad, Well # 20V on February 21, 2019. There is 1 inactive and shut in well on this pad.

Annular vent (8-5/8" x 5-1/2") is open and plumed to the poly tank. 0% combustible gas was detected in the area immediately surrounding the well. Production equipment is removed from location. No obvious concerns or violations are noted at time of inspection. Seneca Resources Corp. Representative - Stephen Gus Simpson, Roustabout Foreman. 2849525 Primary Facility 2019-03-05 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, uncompleted, unconventional well was conducted March 5, 2019. Plants and Goodwin Rig 12 is on location for plug and abandonment along with a cementing unit rigged up to pump bottomhole plugs only. I met with the rig supervisor to discuss the well and operations on the pad. Production casing has not been perforated and has no pressure. A shut in test over the weekend on the intermediate and surface casings indicated similar pressures; approximately 58 PSI. Pressure was bled off and an additional overnight shut-in was conducted Monday night. Overnight intermediate casing pressure was 42 PSI and surface casing pressure as approximately 60+ PSI. A CBL was ran on the production casing to analyze annular integrity and identify potential squeeze intervals. I reviewed the CBL; production casing is cemented to surface and therefore unrecoverable. No large intervals of free pipe were observed. The operator is evaluating their options for remediation, which may include
2854125 Primary Facility 2019-03-12 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, unconventional well was conducted March 12, 2019. At the time of inspection, Plants and Goodwin Rig 12 are on location for plug and abandonment. I met with the plugging consultant to discuss operations on the pad. The crew have just started the process of milling a window in the production casing as part of remedial efforts to address annular migration. They intend to mill an approximately 30' window below the intermediate casing shoe to pump cement out to formation to isolate the Venango sand which is a suspect source of migration. Returns were being monitored; metal shavings observed. Annulus pressure was being monitored; approximately 48 psi. No obvious major issues were observed during this inspection.
2856471 Primary Facility 2019-03-20 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, uncompleted, unconventional well was conducted March 20, 2019. Plants and Goodwin Rig 12 is on location for plug and abandonment. The rig crew have successfully cut window in production casing below surface shoe near suspect gas source and under-reamed cement in the annulus in preparation to cement as part of remedial effort to address annular migration. A shut-in test has seemingly indicated the migration pathway has been intercepted as a result of cutting the window as seen by pressure increase on the production casing which previously had 0 pressure. At the time of inspection, cementers are rigged up and have just finished spotting cement across the window. The return line was shut-in at the BOP and the pump operator applied pressured as a squeeze attempt. No significant injection was observed (~1/4 bbl in, 1/4 bbl back to displacement tank after bleed-off). No obvious issues were observed at this time.
