2696803 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted The weather is 36 Deg. F and sunny at time of inspection. Routine inspection of Alta Resources, LLC., C.O.P. TRACT 285G Pad, Well #1041H, on February 13, 2018.

There are 4 producing wells on this pad. Both annulus vents are closed and not vented at time of inspection. Production by intermediate (5-1/2¿ X 9-5/8¿) annulus vent is shut in to pressure monitor, with pressure reading ¿ 0 PSI, production casing annulus vent is also shut in to pressure monitor, with pressure reading - 246 PSI. The production xmas tree top pressure on the 2-3/8¿ tubing is 145 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2807844 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted On November 28, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 285 Pad G. The site could not be evaluated due to the snow cover.

There are a total of 4 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 4 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment. The secondary containment for the GPU units water level was almost over topping(Photo 3). The Department recommends vacuuming the containment out when weather permits. The liner in the secondary containment for the production fluid tanks had holes in it (Photo 4). The Department recommends replacing the liner.

No violations noted.
