2218022 2013-10-31 No Violations Noted On 10/31/13, I conducted an inspection at the Chevron Appalachiaï¾’s Hall-Hogsett 12, 14, 15H Unit OG Wells site in Luzerne Twp., Fayette County.

The new Act 9 site ID sign was in place and legible. The E&S plan and PPC plan and well permits were in the box.

The access road to the site was in good shape. The site was not guarded.

The site looked stable and had fair vegetation growth. I did not observe any E&S issues.

There is no equipment or pits or sed traps on site.

There are 4 cellars on the pad that are fenced off and they have a containment liner in place around the wells.

The cut slope area of the pad is lacking vegetative growth, but looks stable at this time.

There was no activity at the site today.

2362054 2015-04-13 No Violations Noted The Hall-Hogsett site contained a large area on the slope to the left side of the pad upon entry that exhibited rills and little vegetation. It is recommended that this area be addressed to prevent further accelerated erosion. There were also residual waste containers and a non haz waste barrel on containment that was in disrepair. The containment has multiples holes and a berm breach. Waste barrels should be labeled for contents and dated to comply with storage requirements for waste.
