Telephoned Doug Jones, of Catalyst, and we have agreed to meet on site tomorrow at 9:00 AM.

2629206 Primary Facility 2017-08-11 Routine/Complete Inspection Outstanding Violations - Viols Req'd Met with Doug Jones, Frank DiGnazio, Randy Baker and Ray the pumper at the site. They said that Ray had started working on the tank battery on Wednesday, 8/9/17, and had passed along to Randy Baker, his supervisor, about the spill on Thursday morning, 8/10/17. They had a small excavator and were getting ready to unearth a pipeline within the diked area that they think is the source of the produced fluids. We agreed that they would complete a report detailing the extent of the contamination and submit it to the Department for review, we would then meet and discuss the remediation.

You are hereby requested to follow Department Policy 800-5000-001, Addressing Spills and Releases at Oil & Gas Well Sites or Access Road. This Policy sets forth the procedures that should be followed to resolve the violations associated with the spill/discharge/release noted in this Notice of Violation by using either the Act 2 or the Alternative Remediation Process. A copy of the Policy is attached for your review.

2630679 Primary Facility 2017-08-29 Routine/Complete Inspection Outstanding Violations - Viols Req'd Don Hegburg and I inspected the site and discussed arranging a meeting with Catalyst to discuss their delineation of the contaminated area in and around the tank battery and their proposed remediation of the area.
