Twp. road is slipped at well entrance n ot due to access to the well. Bench at Lewis #! is slipping and developing internal drainage into slip crack, therefore the bench will need reworked.
2700464 Primary Facility 2018-02-22 Routine/Complete Inspection No Violations Noted On Thursday, February 22, 2018, I conducted a routine-complete inspection at the Lewis No. 1 Well Site. The vertical gas well is located in Center Township, Greene County.

The access road and the pad area were in good condition and the site has been stabilized with vegetation. The area is used to graze cattle and cattle were grazing at the time of inspection.

There is no secondary containment around the production tank. The tank appeared to be in good condition with no apparent signs of degradation.

The gas well and associated infrastructure which is surrounded by a fence to keep cattle out, appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.
