1947232 Primary Facility 2010-12-08 Routine/Complete Inspection Viol(s) Noted & Immediately Corrected Rick Palmer and I inspected this site for ESCGP-1 (ESX10-059-0060) Major Modification Application. We found the drawings (plans) were different from site construction (e.g. Rock Construction Entrance, Channels, Pits, etc.) In case of GP-7 (minor road crossing) for the access road, we found several violations. There was no GP-7 permit on site and the site design was not met the conditions (e.g. Depth of fill was higher than 5 ft, the culverts was not inverted 6 inches, etc). They corrected and submitted the evidence on 12/21/2010. GP-7 was also submitted. Scott and Michelle inspected follow-up in January. See the details attached.
1966667 Primary Facility 2011-01-25 Routine/Complete Inspection No Violations Noted The Department was at the above referenced CBM Well to conduct a routine inspection. This well pad contains 2 CBM wells and 1 Marcellus well. The ESCGP-1 was permitted to Coal Gas Recovery and will be responsible for the site BMP's. The stream crossing has a 36 inch culvert and appears to have 18 inches of fill on the pipe. The super silt fence at the toe of the slope appears to be in good condition. The water laying on the site did appear to have an oily sheen of some kind on it. The company man on site said he would take care of the water and pump into the pit. There was some soil pushed into one of the diversion channels on the pad and is blocking the flow of water. Please make sure that all channels are clear so the rock filter can work properly.
2453056 Primary Facility 2016-02-25 Routine/Complete Inspection No Violations Noted The registration number is installed on the well. The well is equipped for production. Production equipment and piping appears to be in good condition. One production tank is located in a diked area. Well is operating. Site appears to be in good condition.
2761234 Primary Facility 2018-08-03 Routine/Complete Inspection No Violations Noted On Friday, August 3, 2018, I conducted a routine-complete inspection at the DFM-3 CBM well which is located in Center Township, Greene County and is accessed through the Grove Well Site.

The access road and the pad area were in good condition.

The site has been stabilized with vegetation.

There is no on-site production fluid storage.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
