Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1955018 Primary Facility 2011-02-15 Complaint Inspection No Violations Noted The Department was at the above referenced well site to conduct a Complaint Inspection. The Department had some potential pollution concerns. The E/S Controls were not in good working order. The silt fence was down in multiple locations. The hillside was slipping on the cut side of the well site. The Department has contacted the Operator and the E/S controls and the slip will be addressed.
1955028 Primary Facility 2011-03-03 Routine/Complete Inspection No Violations Noted Remarks: The Department was at the above referenced well site to conduct a waiver inspection. The Operator is requesting approval for the 100 ft distance restriction. The Department will approve the waiver with comments. The operator must fix the E/S controls and address the slip on the cut side of the site before they can drill the gas well. A 50 ft rock entrance must be constructed before any equipment is brought on this location. The Department met on site with the Operator and their Consultant to discuss the waiver and site conditions. The Operator agreed to take all waste to a landfill because there is not enough room on location to encapsulate waste greater than 100 ft from the stream. The Department will conduct follow-up inspections in the near future to insure the site is in compliance with Oil & Gas Regulations.
