2118249 2012-11-28 In Compliance with Policy Administrative Review of this Incident revealed the following:

A spill incident was reported by the operator, Chevron Appalachia, on 11/26/2012 at the Milesky 1H & 2H well site located in Center Township, Greene County. An approximate 2 gallons of hydraulic oil leaked from a water truckï¾’s pump. The water hauler, Heckman Water Resources (HWR) actually responded by over excavating the surrounding gravel and putting the material in a spill box. Currently (11/28/12) the material is stored on-site until the disposal paperwork is completed.

The spill was reported to the Department on 11/26/2012.

Upon review of all the information in relation to this spill incident, the operator performed the proper spill reporting requirements and has reported sufficient remediation at the area of release in compliance with the Departments current Spill Policy.

2186972 2013-06-20 No Violations Noted This well site was inspected concurrently during inspection of the Milesky well pad (Site: 747628 n�e 748631, inspection number 2186892) in response to operator's request for a well site extension. Site appeared to be equipped for production. Well label for Unit 1H (059-25729) needs updated. No erosion or sedimentation issues were noted.
2211071 2013-09-10 No Violations Noted On September 10, 2013, I stopped at Chevronï¾’s Milesky pad for a routine inspection.

There was a good sign out front with all the pertinent Act 9 information. I found an adequate PPC plan in the mailbox as well as an E&S plan Although they are labeled, the wellheads were not tagged according to the regulations; Chevron needs to address this. The site is well-vegetated, but is not restricted and I saw evidence that a herd of cattle had wandered the pad recently. I am concerned that the cowï¾’s hooves may damage the wellhead containment. Because of the vegetation, some of the E&S controls have been removed and this should be noted on the E&S plan. I found good containment around the production tanks and the road appeared in good shape.
