The site is located in Aleppo Twp., Greene Co. The inspection was related to the request by the operator to terminate the ESCGP1 permit for the site.
The ESCGP1 permit for the site expired in 4/2017, but the operator had submitted a major modification to the original ESCGP1 permit in 6/2015 prior to the expiration the Departments review of the proposed modification extended beyond the expiration of the original ESCGP1 permit. The operator has now submitted a request to terminate the original ESCGP1 permit. The operator has failed to restore the well pad and access road to the original ESCGP1 permit conditions. The operator has failed to remove and restore E&S BMPs (sediment trap). The site is stable with greater than 70% uniform vegetative cover.
