2575921 Primary Facility 2017-03-23 Administrative/File Review In Compliance with Policy According to Mr. Johnson: The well does not produce brine water. The brine tank has an open hatch which filled with rainwater, then froze, causing the valve to break. The brine tank currently has a large block of ice in it. The valve has been plugged.

Per Mr. Johnson: The welltender collected conductivity readings of 170 Microsiemens of both the released fluids and the potentially impacted soils. The brine tank was dripping fluids when the welltender discovered the release. The welltender determined that 13 barrels was lost based on tank readings. The welltender was last at the well site on 2/16/17.

Mr. Johnson stated that Spartan will be on-site today or tomorrow to collect soil samples and fluid samples. The brine tank will be removed.

Mr. Johnson stated that there is no dead or stressed vegetation and that there are no nearby waterways.

The Department recommended to Mr. Johnson that all fluids are contained should there be any potential contaminants in the released fluids and that the operator demo

2575968 Primary Facility 2017-03-24 Administrative/File Review In Compliance with Policy Admin inspection in response to inspection record 2575921 for the reported release of 13 bbl freshwater from a brine tank.

E-mail providing additional details surrounding release received by Department from Operator representative Spencer Johnson. Please see attached.

2577983 Primary Facility 2017-03-27 Incident- Response to Accident or Event In Compliance with Policy Inspection conducted in response to admin inspection 2575921 for the reported release of 13 bbl freshwater from brine tank.

During this inspection, Department did not observe any visibly impacted soils or any dead and/or stressed vegetation. Per operator, fluid and soil samples were collected to demonstrate that a release of only freshwater occurred and the results will be provided to the Department upon receipt. This inspection is pending upon the receipt and review of the sample results.

At time of inspection, well drilled and appears equipped for production. Well label present. Department recommends operator replace well tag on well in a legible, visible, and permanent manner per current regulations.

2584016 Primary Facility 2017-04-10 Administrative/File Review No Violations Noted Admin inspection conducted in response to inspection record 2577983 for the reported release of 13 bbl freshwater.

Sample results received via email by operator representative Spencer Johnson. See attached.

UPDATE: 4/11/2017 At 1044, the Department contacted Mr. Spencer via phone to determine what exactly was sampled, the sample location, and the soil conductivity readings collected. Mr. Johnson stated that he would verify this information and promptly follow-up with the Department. At 1053, Mr. Spencer contacted the Department via phone to confirm that the sample was collected from the fluids within the brine tank. Soil samples were not collected based on site conditions (i.e. No visual evidence of any releases observed) and low soil conductivity readings. At 1111, Mr. Johnson provided the Department with the soil conductivity readings via email. Mr. Johnson reported that the soil probe read 0.05 micro Siemens.
