2114327 Primary Facility 2012-11-06 Incident- Response to Accident or Event Violation(s) Noted nspection result of incident notification at 1435 on 11/5/12 of a release of 30 barrels of production water from cracked line between two brine tanks. Well TDed 7/18/2003. Well tag present. No accelerated erosion or sedimentation issues noted on site or access road. While onsite spoke to Todd Houser, XTO. Cracked (production) line was discovered at 1400 hrs on 11/5/12, well (to knowledge) last visited by operator personnel Friday Nov 2nd or Saturday Nov 3rd. Release not believed to have reached waters of the commonwealth. It appears area of impact has been delineated (spill contained) by operator and excavation of contaminated soils has begun. Currently, 5 1/2 roll off boxes (of the 8 onsite) have been filled with excavated "contaminated" soils. Samples from waste contained in roll off boxes were collected by McCutcheon. Mr. Houser was onsite taking soil conductivity readings of impacted areas.

2834001 Primary Facility 2019-01-29 Routine/Complete Inspection No Violations Noted On 1/29/19 at 0902 hrs., I conducted a routine inspection of Alliance Petroleum Corp.'s Mumau 2 well (API 063-33347) in Grant Twp., Indiana Co. Snow cover prevented proper evaluation of site stabilization. The pad appears to be restored and permanently stabilized. The well is equipped for production and a site ID sign is posted at the entrance to the pad. A well tag is not present at the well head, and one should be installed prior to re-inspection of the site. There are no violations being cited for this inspection.
