1844190 Primary Facility 2009-11-16 Drilling/Alteration No Violations Noted According to records, well permit issued on 4/10/09. Conductor (13 3/8¿) set at 54¿. First string (9 5/8¿) set at 225¿ with 100 sks Class A 9 bbl return. Second string (7¿) set at 1215¿ with 150 sks lite and 75 sks Class A. No voids or water detected on log. Stratigraphy consisted of shale, sandstone, coal and limestone. Two pits completed not aligned with plastic. No freeboard or IW discharge issues. Well sign and E&S plan located at entrance way of site pad. No E&S issues at the site. At the time of site visit, geophysical was in the process of completion. PC needed to set the 4.5¿ at 3812¿. TD was at 3838¿.
1849167 Primary Facility 2009-12-03 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled by not yet in production. The site was properly signed with E&S plan. I encourage operator to continue closely monitoring access road entrance and all BMPs (filter fence, need for additional rock) until final restoration activities have been completed in order to avoid any possible accelerated E&S concerns that may arise. Noted 3 pits onsite, two were lined and contained fluids, one was unlined. Again, I am also encouraging operator to closely monitor pit freeboard at all times as levels(s) were approaching the 2ft freeboard requirement. No IW issues observed.
