Observed three below grade pits located onsite and one was in use as a drill pit. One of the other pits was being filled with fresh water. Both pits had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation with respect to the access road or well-site. As the snow continues to melt the operator should evaluate the erosion controls to insure that they are in good condition and will to continue to function properly until well development activities are completed.

1885595 Primary Facility 2010-05-18 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled not yet set for production. Well site was properly signed with E&S plan at site entrance. Noted three below grade pits were installed onsite and maintained more than a 2ft of lined freeboard on all pits. Soil pile was temporarily seeded and mulched. No accelerated erosion or sedimentation concerns were noted in relation to well site or access road, as all temporary bmp¿s were properly installed and functioning as outlined in E&S plan. Operator should continue to monitor freeboard levels on all pits, as well as all E&S controls until final restoration activities have been completed and site is permanently stabilized. 1900992 Primary Facility 2010-07-27 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well sign and E&S plan located at the entrance of site. Met Jim Braun at the site. Universal started Hydro frac of Rau Unit 2H well and completed Stage 1 of 8 stages. At the time, no signs of H2S odors according to Atlas.
1923356 Primary Facility 2010-10-27 Routine/Complete Inspection No Violations Noted During this inspection, well drillied April 2010, and fraced July 2010. Proper signage and ES plan available. Appears site has been recently seeded and mulched as<70% uniform vegetative growth is observed. Four tanks on site are surrounded by dike. No accelerated erosion or sedimentation issues observed. No H2S odors detected by operator during drilling or fracing operations. Tempoary bmp¿s installed as outlined in plan and functioning. Upon final restoration (permanent stabilization, >70% uniform vegetative growth) operator may remove all temporary bmps, until they all should be closely monitored. Access road entrance stable.
1939432 Primary Facility 2010-12-21 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well tag present. Site has been restored. Well head consisted of 8¿x 4¿ x 2¿ and manifold to both wells. Wells are in production. Production equipment consisted of separator (200 psi) , heater and 4 ¿ 200 bbl brine tank (located in dike area). No signs of gas leak around equipment. Well head located in security fence

Well head surrounded by fencing. Four brine tanks surrounded by dike. Now IW or leaks detected. No gas or H2S odors detected. Well is within mile radius of a well that encountered a high levels H2S. Pad and access road sufficiently rocked. Site appears to be restored as adequate vegetation observed and no accelerated erosion or sedimentation issues seen. Note: some snow accumulation located onsite. Operator should continue to maintain and monitor site through winter and spring thaw to ensure site is adequately restored to permanently resist erosion. 2834076 Primary Facility 2019-01-29 Routine/Complete Inspection No Violations Noted On 1/29/19 at 1149 hrs., I conducted a routine inspection of Diversified Oil & Gas, LLC's RAU Unit 2H well (API 063-36738) in West Wheatfield Twp., Indiana Co. Snow cover prevented proper evaluation of site stabilization. The pad appears to be restored and permanently stabilized. The well is equipped for production and a site ID sign is posted. A well tag is present at the well head. There are 4 production fluid tanks located within adequate secondary containment. There are no violations noted for this inspection.
