Conductor (13 3/8¿) set at 34 ft. Stratigraphy consisted of Sh, SS and coal. Coal/mine noted at 324 ft. PC setting 9 5/8¿ at 378 ft. Water noted at 50¿, 70¿ and 200¿. Current depth was at 290 ft. E&S controls were present. Based on site conditions, no indication of IW issues at the site. No E&S issues at the site.
1905362 Primary Facility 2010-08-11 Routine/Complete Inspection No Violations Noted At the time of inspection, well was drilled not yet equipped for production. Rig onsite at time of inspection. Site was properly signed with E&S plan. Access road entrance adequately rocked and stable, although recommend that operator add rock to culverts and outlets. Noted two below grade pits onsite, both of which sustained the required two of lined freeboard and appeared structurally sound. Noted minor E&S issues behind soil pile, operator could temporarily seed pile to resist erosion. Site was surrounded by corn fields. Noted, that the operator plans to landform cuttings. No IW issues were observed.

Recommend operator add additional rock to area in order to reduce the potential for a possible pollution event. No accelerated erosion /sedimentation or industrial waste issues noted onsite.

ON 9/24/15 AT 1000, A RELEASE OF 8 BBL BRINE WATER WAS REPORTED TO SWRO REPRESENTATIVE CAROL STACOVIAK BY OPERATOR REPRESENTATIVE ERIK GREENERT. RELEASE OCCURRED AND/OR DISCOVERED ON 9/24/15 AT 0915. NO WATERWAYS REPORTED TO BE AFFECTED.
2410537 Primary Facility 2015-09-24 Administrative/File Review In Compliance with Policy Admin inspt in response to record 2409995 for reportable release of 8bbl brine water.At 1645, Dept spoke to Opr rep E.Greenert via phone.Per Mr. Greenert:8bbl brine water release occurred when BT cracked above bulkhead fitting.Spill discovered today at 0915,subsequently reported to SWRO at 1002.Mr. Greenert believes well tender last at well last week, but needs to verify.Site of release in corn field and area of impact is approx 60'x30'.No waterways reported to be impacted.Excavating is ongoing at time of call.Mr. Greenert believes they may be near encapsulated drill pit; however, it has not been compromised and it is not verified at this time whether or not this is encapsulated drill pit.Department recommended that Opr determine encapsulated drill pit location prior to excavating any deeper in this possible area of concern in order to prevent potentially compromising any encapsulated materials.Opr appears to be ICP at this time.A FUI will be conducted to assess any environmental impact and/or issue any associated viols as necessary.
2413476 Primary Facility 2015-09-25 Incident- Response to Accident or Event Violation(s) Noted Inspection performed in response to reported release of 8 barrels of brine-produced water by the Operator on 9/24/2015 at 10:38am (Inspection ID# 2409995). Clean-up procedures have been initiated by Operator. The compromised brine tank has been disconnected from wellhead apparatus and moved to edge of well site. Approximate 60’x30’ area of impact has been excavated of immediate impacted material. A smaller 10’x10’x10’ area where the compromised brine tank was previously located has also been excavated. All excavated materials stored in 8 securely covered roll off containers. Well site located approximately 250ft up gradient of Muddy Creek. At the time of inspection no evidence of the release impacting the waterways was noted. At the time of inspection, potentially encapsulated drill pit does not appear to be compromised as referenced in inspection ID 2410537. Perimeter temporary E&S BMPs properly installed and site temporarily stabilized.

At time of inspection, well appears equipped for production. No brine tank on-site. According to records, drilling occurred 7/27/2010- 8/02/2010. Well tag present, legible.

Area of release appears restored to pre-existing conditions (i.e. ag field). Access road stabilized with rock. No accelerated erosion and/or sedimentation issues observed during this inspection. In accordance with the Spill Policy, the 15 day initial written report was received on 10/19/2015 notifying the Department that the Operator will be utilizing Act 2 for the remediation of this release. The Final Report was received by the ECB program on 11/30/2015 and subsequently approved on 1/28/2016. No violations noted during this inspection.
