2017853 2011-11-09 No Violations Noted Site visit was in response to incident notification by Nick Staffieri (Chevron) to Ed Ritzer (PA DEP Water Qualiy Specialist Supervisor at 07:43 on 11-9-11.Spill occurred do to the following: Haliburton had been cementing the well. Well was being cemented at 8 gpm. This rate caused an overflow of oil based mud at the well head. This overflow was not immediately recognized. Multiple levels of containment were breeched and the mud made it to the surface of the pad. A small area of stone on top of felt lined material was contaminated with this material. Most of the mud had been removed at the time of inspection either by vac'ing or through absorbents. Crews continued to lay absorbent material on any spilled material that had not yet been removed. Absorbent material was then being removed for proper disposal.

Previous incident response was pending. Follow-up on 2-23-12 resulted in NOVIO.

2020929 2011-11-18 Pending Site visit was follow-up to 11-9-11 incident inspection to mud based oil spill.

Rig had been moved to second hole on pad. Secondary containment had been fixed and reinstalled. Site was frozen and muddy, but did not appear to have any residual mud left on the pad. Site will be revisited when rig is removed to insure no residual oil is left on the pad. BMPs were in place and working properly. No E&S issues. Site inspection was conducted with Jesse Olson (PA DEP Inspector). Pat Patterson was company man on-site.

2034311 2012-01-11 No Violations Noted During this inspection, Site Id sign located at access road entrance. ES plan, PPC and ESCGP-1 located onsite. Pioneer Rig 60 onsite actively drilling 9H , 8H previously drilled and top hole drilled on 10H. Pad lined with secondary containment, which appears structurally sound. Temporary bmp's installed/functioning as per ES plan. No accelerated erosion or sedimentation issue noted. Access road stable. Site gated and protected by security guard. No violations noted during this inspection. Although as noted previously, once containment is removed a follow up inspection will be conducted to evaluate what/if any residual impact resulted from 11-8-11 release. Operator shall continue to closely monitor and maintain all bmp's until final permanent site restoration is achieved.

2037049 2012-01-24 No Violations Noted During this inspection, Site Id sign located at access road entrance. ES plan, PPC and ESCGP-1 located onsite. Pioneer Rig 60 onsite actively drilling 9H,(Inspt Id # 2037025) 8H previously drilled (drill deeper) and top hole drilled on 10H.

Pad lined with secondary containment. Temporary bmp's installed/functioning as per ES plan. No accelerated erosion or sedimentation issue noted, although deparment observed areas of ponding ponding water on well pad. As a result department is reccommending operator closely monitor and maintain areas and add additional rock as needed in order to prevent a possible pollution or erosion event. Operator shall continue to closely monitor and maintain all bmp's until final permanent site restoration is achieved. Access road stable. Site gated and protected by security guard. No violations noted during this inspection. Although as noted previously (Inspt Id #2020929), once containment is removed a follow up inspection will be conducted to evaluate what/if any residual impact resulted from 11-18-11 release.

Well(s) drilled, not yet equipped for production. Secondary containment has been removed from pad. No accelerated erosion or sedimentation issues noted on well pad or access road. One drill pit located onsite with adequate freeboard, cuttings and liquid fraction present in pit. Operator plans to re-rock well pad and removed drill pit upon completion of frac operations. Although previously disturbed areas have been seed/mulch with observed growth operator shall continue to montior and maintian all bmp's until final permanent restoration achieved. No violations noted during this inspection

Curtis Small (Chevron) was on site. Rig was moved off site. Liner was removed. No residual mud or other materials were visible at or around the well head. Site ID Sign Present, Permit(s), ES plan, ESCGP-1, and PPC Plan Present.

At the time of this inspection it appears operator has removed impacted soils and placed it along with secondary containment liner in a secured roll off box. No environmental impact visible during this inspection. It is believed (per operator) that 99% of the material released was freshwater, mixture of friction reduce was result of residual that "may" have been left in truck tank. No violations noted during this inspection.

One uncovered dunmpster onsite. Two frac tanks located onsite within secondary containment, lines leading to lined below grade pit. No accelerated erosion or sedimentation issues noted at the time of this inspection. Reminder that operator has 9 months from completion of drilling of last well, on multi well pad, to restore unless other approval is demeed from the Department as per current regulations. No violations noted at the time of this inspection.

Site monitored by security guards. Multi Well pad, Three wells drilled not yet in production. All labeling present on well heads. Access road entrance paved. Well site stabilized with adequate rock and vegetative mats installed in drainage ditches. Previously disturbed areas adequately vegetated and temporarily stabilized. Rig onsite drilling out plugs. Operator plans to flow back into tanks and/or pit. Currently pit is structurally sound, but is approaching freeboard. Operator agreed to pump out pit contents prior to beginning flow back operations. Secondary containment placed underneath rig appears. No accelerated erosion or sedimentation issues noted at the time of this inspection. Reminder that operator has 9 months from completion of drilling of last well (9H), on multi well pad to restore unless other approval is deemed from the Department as per current regulations. No violations noted at the time of this inspection.

2109185 2012-10-15 No Violations Noted Inspection result of request for restoration extension. Department received a request for restoration extension on October 4, 2012 (see attached) via email. As per regulation the current (9 month) restoration Deadline is October 26, 2012, based on completion of drilling of last well, on multi well pad, of 1/26/12. At the time of today's inspection, site ID sign, ES plan, ESCGP-1, permit, and PPC plan located onsite. Site monitored by 24-hr security. Wells drilled not yet in production. Site has been temporally stabilized with bmp's and/or rock. No accelerated erosion or sedimentation concerns noted. One below grade lined drill pit onsite. During this inspection Waste Management (independent contractor) and Chevron personnel were actively removing contents (liquid and cuttings) from pit via vac truck. Liner appears structurally sound and freeboard level maintained. No industrial waste concerns were noted at the time of this inspection. It appears that water and liner have been removed from freshwater impoundment. No viols. Restr ext granted

Site temporarily stabilized, department previously granted restoration extension until April 26, 2014. Pad adequately rocked. No accelerated erosion or sedimentation issues noted at the time of this inspection, although site is snow covered.
