1920376 Primary Facility 2010-10-18 Routine/Complete Inspection In Compliance with Policy This inspection is in response to an OG -57 waiver submitted by the operator to construct a well <200ft from a stream, wetland, and/or body of water. Reviewed ES Plan. Well staked and approximately 147¿ from closest point of intermittent stream (UNT to Conemaugh River). Operator should implement and maintain all bmps to ensure protection of the stream and associated lands. It was also noted in plan that operator had plans to encapsulate drill cuttings, but under 78.62(a)(7) in order to encapsulate the pit must be >200ft from a stream, body of water, or wetland. An alternate method of disposal is recommended, but in order to encapsulate pit must be >200ft away from closest point of stream. Operator is also investigating the solidification process. Possible relocation of access road was also discussed in order to limit the amount of disturbance and to relocate the access road further from stream; operator must first discuss with surface owner and then notify dept of final decision. Waiver approved with above comments
1933030 Primary Facility 2010-12-09 Drilling/Alteration No Violations Noted According to records, permit granted 10/12/10. Currently, PC Rig #3 contracted by Penn View. Filter fence located topographically down gradient of site pad. Pit was covered with 20 ml plastic. Permit present. No freeboard issues. Conductor pipe (13 3/8¿) set to 40 ft (sanded). Due to elevated water issues, PC going to set 9 5/8¿ pipe at 252 ft. Current depth was 250 ft.. Stratigraphy consisted of shale, sandstone, and limestone. Water recorded at 190 ft. During site inspection, no E&S or IW issues at the site.
1934189 Primary Facility 2010-12-14 Drilling/Alteration No Violations Noted At the time of inspection, no E&S and IW issues. PNG contracted Union to complete storage well. Inspected site. Union set 20¿ conductor to 42 ft. and 13 3/8¿ pipe to 144 ft. 9 5/8¿ pipe surface casing was set at 1960 ft. Current depth was at 7988 ft (located in the chert zone). PNG revised TD to 8200 ft. Filter fence was installed. Pit covered with 20 mil plastic with discharge line from rig to pit secured with cement blocks. No freeboard issues
1939897 Primary Facility 2010-12-20 Routine/Complete Inspection No Violations Noted At the time of this inspection, site properly signed with ES plan. Waiver on distance to a stream was granted, with conditions on 10/18/10. Well drilled, but not equipped for production. Access road stable and moved according from original plans with dept approval. Bmp¿s installed and functioning according to plan. Recommend operator continue to routinely monitor and maintain through spring thaw and until final restoration activities are completed. One below grade pit onsite, lined with adequate freeboard. Awaiting operator response on final disposal method chosen for pit contents. Accompanied by WQS Marc Maestra.
1955571 Primary Facility 2011-03-08 Routine/Complete Inspection No Violations Noted *Waiver Site*- At the time of inspection, well drill in production. Well tag present. Brine tank ok. Temporary bmp¿s installed and functioning according to plan. Site has not yet been seeded/mulched, but pit(s) have been reclaimed as dept recommended that pits were reclaimed as soon as possible due to proximity to the stream. Minor ES issues were noted with access road. It is recommended that the operator add additional rock to ensure stability of roadway, as well as to crown roadway to ensure water from culvert pipe flows along rocked culvert, which may need additional rock to prevent a possible pollution event to the nearby stream and springs. No accelerated erosion or sedimentation issues were noted on site. Now IW issues. Operator should continue to closely monitor and maintain all bmp¿s until final restoration is completed (within 9 months of drilling completion).
2026132 Primary Facility 2011-12-13 Site Restoration No Violations Noted *Waiver Site* Well spudded 12/08/10. Drilling completed 12/14/10. Well drilled, equipped, and in production. Well tag present. Brine tank appears structurally sound. Access road stable. Temporary bmp's in place, maintained, and functioning as outlined in ES plan. No accelerated erosion or sedimentation issues noted. Approximately 70% vegetative covered observed in previously disturbed areas; therefore, well site appears restored. Department recommends operator seed and mulch in the spring to ensure uniform vegetative cover is maintained, especially along cut slope. No violations noted at the time of this inspection.
