I entered the site at 10:30. At the time of the inspection, the George Hagemeyer A 1H (081-20692), 2H (081-20937), 3H (081-20398), 4H (081-20938), 5H (081-20698), and 6H (081-20939). ESX10-081-0038 was amended on 7/17/2015 to address PCSM (Post Construction Stormwater Management) and well site restoration. Currently, the site remains at the originally permitted size and the amended plan has not yet been implemented.

The most recent update to the well site restoration assessment Anadarko provided on 12/22/2016 indicates the PCSM for the site was to be implemented in 2016. The WSRR (Well Site Restoration Report) and 2-year extension were scheduled to be submitted to the Department in 2017. I recommend implementing the permitted plan. No Violations Noted.

I entered the site at 14:05. At the time of the inspection, the George Hagemeyer A 1H (081-20692), 2H (081-20937), 3H (081-20398), 4H (081-20938), 5H (081-20698), and 6H (081-20939) were in production. ESX10-081-0038 was amended on 7/17/2015 to address PCSM (Post Construction Stormwater Management) and well site restoration. The PCSM/Site Restoration plan has been implemented and the instrument recorded on the deed. The site is permanently stabilized and is in good condition. While the pad surface has not been reduced in size, the permit modification clarifies the use of the remaining well pad area and has minimized the overall footprint of the site. I recommend the Notice of Termination for this site be approved. I also contacted the operator and requested that the Well Site Restoration Reports be submitted for the wells on this site.

4/6/2017: On 4/6/2017, the operator submitted a Well Site Restoration Report (WSRR) as requested in the 3/23/3017 inspection. T
