2412778 | Primary Facility | 2015-09-28 | Routine/Complete Inspection | No Violations Noted | Inspection on wells in production. Permit number on well tag incorrect. No violations noted at this time. |
2482619 | Primary Facility | 2016-05-06 | Routine/Complete Inspection | No Violations Noted | At the time of the inspection, the Gayla Loch 1H (081-20909), 2H (081-20952), 3H (081-20910), 4H (081-20911), and 5H (081-20912) were in production. The well site has not been restored and the sedimentation basins have not been converted to infiltration basins based on the PCSM (Post Construction Stormwater Management) plans. Rock Infiltration sumps along the access road have been installed as designed in the PCSM plans. The revised restoration assessment acknowledges the well site has not been restored and drilling ended on 5/5/2014. The well pad remains at its original size and no well permits have been issued since 2012. The well site has not been restored based on the criteria above and the current ESCGP expires on 12/6/2016. I recommend restoring the well site or implementing the PCSM and submitting a 2-year extension. Overall, the site is currently permanently stabilized (with both vegetation and rock) but has not been restored per §3216(a) of the Oil and Gas Act. No Violations Noted. |
2662551 | Primary Facility | 2017-11-20 | Routine/Complete Inspection | No Violations Noted | Weather 38 Deg. F, sunny and windy. Well has a digital gauge with 320 PSI production pressure. Also backside pressure on the tubing digital gauge shows 415 PSI. 0% combustible gas detected at the time of inspection. No annulus vents observed during inspection. |
2681130 | Primary Facility | 2018-01-03 | Routine/Complete Inspection | No Violations Noted | The access road (including stream crossings and drainage features) from SR 184 to the well pad itself was in good condition and well maintained. A few inches of snow was covering the entire site. On the well pad, five wells were in production. Sand separators remain installed at the production units and both (for each well) are in containment. No staining was visible around any of the production equipment. Methanol, foamer, and defoamer were stored (covered and in containment) near the wellheads for injection.
The ESCGP permit has expired and Post Construction Stormwater Management (PCSM) has not yet been permitted or implemented. I recommend re-permitting the site and following the COA which indicates this site will be restored in 2019. No violations noted. |
2793641 | Primary Facility | 2018-10-24 | Routine/Complete Inspection | No Violations Noted | Five wells staged for production phase. Five each sand and water separators. No leaks detected from production equipment. Methanol tanks located near well heads. No violations noted. |
