Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2135998 Primary Facility 2013-01-29 Routine/Complete Inspection No Violations Noted Preoperational insp. requirement fulfilled, still need to be stabilized when weather permits. NoVio
2147990 Primary Facility 2013-03-04 Routine/Complete Inspection No Violations Noted
2159862 Primary Facility 2013-04-18 Routine/Complete Inspection No Violations Noted
2277737 Primary Facility 2014-06-06 Incident- Response to Accident or Event No Violations Noted On 6/6/2014, Andrea Mullen (DEP) and I met with Mr. Subra on the well pad and containment had been removed around the spill area. A failed seam between old containment and new containment was the suspected cause of the brine release. Mr. Subra was directing the excavation/remediation of the pad by soil conductivity. I asked Mr. Subra to test no less than 20 locations throughout the spill area which were still wet. No locations were found where soil conductivity was over 1,000 umhos/cm. Excavated pad material was being stockpiled (on liner) for disposal. Violations Noted and Immediately Corrected.
2471969 Primary Facility 2016-03-30 Routine/Complete Inspection No Violations Noted Routein inspection of well in production. No violations noted.
2517281 Primary Facility 2016-09-07 Routine/Complete Inspection No Violations Noted Arrived @ 11:50. Weather: sunny, light breeze, low humidity. This was a routine inspection. There were no personnel on site. There were two producing wells. Cellars were gravel filled:

Null Bobst Unit 1H Top gauge: 620 psi (0-1500 psi analog), 13-3/8” x 9-5/8”: no combustible gas detected (Altair 5X gas meter calibrated 09/07/16), 9-5/8” x 5-1/2”: plumbed to 225-gallon polytank. no combustible gas at polytank vent. Null Bobst Unit 5H No top gauge, 5-1/2”: 600 psi (0-1000 psi analog), ), no 13-3/8” x 9-5/8: vent., 9-5/8” x 5-1/2”: plumbed to 225-gallon polytank, no combustible gas detected. Left location @ 12:20.

2572078 Primary Facility 2017-03-08 Routine/Complete Inspection No Violations Noted Arrived @ 13:49. Weather: windy. This was a routine inspection. There were no personnel on site. There were two producing wells. Cellars were gravel filled:

Null Bobst Unit 1H Top gauge: 640 psi (0-1500 psi analog), 13-3/8” x 9-5/8”: no combustible gas detected (Altair 5X gas meter calibrated 03/06/17), 9-5/8” x 5-1/2”: plumbed to 225-gallon polytank. no combustible gas at polytank vent. Null Bobst Unit 5H Top gauge: 600 psi (0-1000 psi analog), ), no 13-3/8” x 9-5/8: vent., 9-5/8” x 5-1/2”: 25% combustible gas detected. Left location @ 14:00.

2791489 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted Two wells staged for production phase. Two each water separators. No leaks detected from production equipment. No violations noted.
