Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2198902 Primary Facility 2013-08-26 Routine/Complete Inspection No Violations Noted
2206909 Primary Facility 2013-09-24 Routine/Complete Inspection No Violations Noted
2592502 Primary Facility 2017-04-27 Routine/Complete Inspection No Violations Noted Inspection of 11 3/4" x 8 5/8" annulus vent on well. 0% methane detected at time of inspection. No violations noted at time of inspection.
2707847 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted A routine inspection of this unconventional well pad was conducted March 15, 2018. The purpose of inspection was to verify the 1-H is plugged because no plugging certificate was found and the well currently holds an Active status. At the time of inspection, the 1H appeared to be a plugged 20" conductor only, with a plate welded across the top and a 2" candy cane vent as a plugged well monument with marker. No obvious issues were observed. There were 3 producing wells on the pad. I conducted a visual inspection of these three producing wells and did not discover any obvious issues.

Please submit a plugging certificate for the 1-H if it is in fact permanently plugged at your convenience.

2775330 Primary Facility 2018-09-14 Routine/Complete Inspection No Violations Noted Four wells staged for production phase. Four each water separators. Sand separator before GPU. No leaks detected from production equipment. Methanol tank located near well heads. No violations noted.
2856710 Primary Facility 2019-03-21 Routine/Complete Inspection No Violations Noted Three wells staged for production. One plugged well (081-21154). Three GPUs within polyurethane/foam containment. No leaks detected from production equipment. Methanol tanks and foam tanks staged near well heads. No leaks.
