Previously disturbed soils have been seeded and stabilized. Vegetation is beginning to grow throughout the site. Overall, the site is in good condition and no E&S issues were observed. Please be mindful of the sharp curves on the access road near the entrance and plan to maintain accordingly. It's probable that truck traffic may have difficulty staying on the road.

2728675 Primary Facility 2018-05-03 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection of PGE's COP Tract 322 Pad E on May 1, 2018 with company man Matt Cobine.

A fracking operation was going on during my inspection. Crews were in the process of fracking wells 081-21330 and 081-213331. All fracking equipment, diesel tanks, etc. were stationed on secondary containment with a berm. No sign of rips were observed in the containment. No water was observed on the containment. Overall, the containment looked great.

Above ground double walled 4" HDEP water lines were pumping water from nearby PGE freshwater impoundments (FWI). The water line configuration is the following: a 4" double walled HDEP pipe is running above ground from PGEs COP Tract 293 A FWI and discharging into COP Tract 293 WI 2 impoundment. Water from COP Tract 293 WI 2 is being withdrawn from this impoundment and going into the buried water line running paralleling Okome Rd. and this waterline is discharging into Tract 322 FWI. 2738656 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted I met Matt Covine when I arrived. Frac was finished, activity on location included moving metal drip pans/rig mats and cleaning out Baker tanks. A Matt believes that flowback operations will not begin until mid-July. Site was clean, most containment had been removed. Near the well heads, moving the large drip pans had exposed a small amount (2-3 gallons) of water on the liner; conductance readings were over my meter's limit (19.99 mS/cm). Matt was aware of the water and had a vac truck ready to clean the area.
