2665746 Primary Facility 2017-11-21 Routine/Complete Inspection No Violations Noted A routine inspection was conducted at the JKLM Reese Hollow 115 well pad (ESX29-105-16-0003) located in Sweden Twp., Potter Co. on November 21, 2017 @1235 hours. Keane Group, Inc. was onsite conducting stimulation activities on the 6HU well (API#10521874). Scott West, a Consultant for JKLM, was present onsite for the inspection. Mr. West contacted Mr. Jon Manuel, he arrived onsite after fluid exhibiting high conductivity was discovered off containment.

Fluid was observed along the western side of the pad containment. The fluid level was up to the top of the rig matting. An Oakton Multi-Parameter PCSTestr 35 was used to test the field conductivity of the fluid in containment at "Or" or >20.00 mS/cm (see Photo#1). Photos #2 and #3 show the level of fluid in the primary containment. A puddle just off containment (see photo #4), on the west side of the pad, had a field tested conductivity of "Or" or >20.00 mS/cm (see photo #5).

2667096 Primary Facility 2017-12-04 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was already stimulated. Hydraulic fracturing(stimulation) was started on 11/14/17 and completed on 12/2/17. No issues or problems were noted during this stimulation.
2668886 Primary Facility 2017-12-07 Routine/Complete Inspection No Violations Noted I arrived on site with Clark Smith of the Environmental Cleanups program and was met by Tyler McConnell of JKLM. We were there to follow up on the containment violations and release noted in an 11/21 inspection by Elizabeth Niles, WQS for the Oil & Gas program. We proceeded to the northwest corner of the pad to the level spreader and sediment trap where Ellie noted the high conductance fluid both in and outside the containment. We noted that the sediment trap had obviously been excavated to an unspecified depth; the water in the trap showed conductance of .63 mS/cm and had frozen enough to walk on. A small amount of slushy water (1-2 gallons) in the containment corner had conductance that read as "OR" on my meter (over 20 mS/cm). Tyler pointed out a vac truck in this area, and told us they had one on the opposite corner as well, as both spots were low spots where water collects. Tyler informed me that JKLM had had 2 of their employees on location during all operations since the incident, as well as uti
2679822 Primary Facility 2018-01-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this well had been hydraulically fractured(stimulated). This well is not producing and is not connected to a gas sales line. The 9 5/8" x 13 3/8" annulus is vented and open and was tested for gas with a gas meter with no gas detected. The 5 1/2" x 9 5/8" annulus was also vented and was closed. This annulus was also tested with no gas detected. There were no violations noted at this inspection.
2694519 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the JKLM Reese Hollow 115 well pad located in Sweden Twp., Potter Co. on 2/12/2018 @1130 hours. Scott West, JKLM was present for the inspection. JKLM is in the process of pulling up the pad-wide containment. Approximately ¼ of the containment was remaining during the inspection. A large pile of snow was observed stockpiled on the remaining containment. Mr. West stated they are using two gas busters onsite to melt the snow. The recovered liquids are being stored in the Aboveground Storage Tank (AST) onsite, awaiting transfer to the frack at the Headwaters 145 site. Any recovered solids are being placed in residual waste containers awaiting transfer to a disposal facility.

Due to recent weather the pad surface is a mix of large puddles, ice and snow. The field conductivity of the large puddles at the center of the pad were ~2.0 mS/cm. A vac truck was in the process of removing the puddles. The standing fluid in the remaining containment had a field conducti

2703335 Primary Facility 2018-03-05 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not in production. This well was temp. shut in for work being done on the annulus vents. The annulus vent lines on the following annulus 20x13,13x9 and 9x5 were being put in place on the wellhead assembly at the time of this inspection.

2721165 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted Some activity on pad when I arrived. Many puddles present on the pad, with conductivity ranging from .35-.90 mS/cm. AST still present on the pad, filled to within 8-9' of the top. Some water had collected in the secondary tank (4-6"); I recommend that it be vacced ASAP to maintain leak detection in this tank. I was unable to locate any well tags; I recommend that tags be affixed ASAP. If tags were present, I recommend that the tags be made more visible.

2742360 Primary Facility 2018-06-14 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the JKLM – Reese Hollow 115 well pad located in Sweden Twp, Potter Co. on 6/14/2018 @1100 hours. The weather was 70 degrees, sunny, 38% humidity and winds WNW 20 mph. The site appears to be setup for flowback activity. The weekly E&S site inspection was last conducted on 6/14/2018 with the comment “housekeeping remains an issue”.

As of today’s inspection, numerous puddles were observed on the pad surface. Standing water was observed on the pad surface along the edge of the gas buster containment. The Electrical Conductivity was field measured With an Oakton Multiparameter PCSTestr and found to be 18.90mS/cm (see attached photo #3 and #4). A smaller “duck pond” containment was located ~1’ away from the gas buster containment. Fluid in the duck pond had a field measured conductivity of 6.72mS/cm(see photo #5). Fluid in the gas buster containment had a field measured conductivity of “Or” or >20.00 mS/cm (see photo #6). The volume is unknown, but it is estimated the rel

2748288 Primary Facility 2018-06-27 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The Flow back process(cleaning out the well of excess hydraulic fracturing water) had been completed. The annulus vent 13 3/8" x 18 5/8" detected a combustable gas reading. During a discussion with Jake Long ,a JKLM Engineer, he informed me that as a result of high pressures and temperatures going through the well during flow back it is not uncommon to see a combustable gas reading on different annulus vents until the well stabilizes the high pressures and temperatures. The gas %(percentage) reading is in the findings section of this inspection. Annulus vent monitoring will be ongoing until the well stabilizes its pressures and temperatures.
2759028 Primary Facility 2018-07-30 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. Flow back, the last part of well development before the well is put intoproduction ended on 6-12-18. Combustible gas was detected on the 13 3/8"x18 5/8" and the 9 5/8" x 13 3/8" annulus vents. This is not uncommon after flow back and going into production as residual flow back will continue for an unspecified amount of time . No violations were noted at this inspection.
2763187 Primary Facility 2018-08-09 Compliance Evaluation No Violations Noted The DEP conducted a Follow-Up Inspection (FUI) at the JKLM Reese Hollow 115 well site located in Sweden Twp., Potter Co. on 8/9/2018 @1030 hours. The weather was 74°, Sunny, Humidity 62%, winds WNW 8mph. Korey Starkey, DEP Env Trainee was present for the inspection.

The well pad is currently setup for flowback activities. A damp area and a small puddle were observed along the front edge of the gas buster containment. The electrical conductivity was field measured with an Oakton PCSTestr 50 and found to be 2.2 mS/cm(Photo#3). Fluid in the gas buster containment had a field measured conductivity of 1700 µS/cm. Numerous patches were observed in the gas buster containment(Photo#4). A vac truck driver onsite was notified of the puddle, exhibiting elevated conductivity, along the front edge of the gas buster containment and the fluid was removed. Puddles were observed around the “duck ponds” adjacent to the production fluid tank containment (Photo#5,6). The electrical conductivity was field measured with an Oak

2765888 Primary Facility 2018-08-20 Routine/Complete Inspection Pending Truck traffic on the site was steady, with water trucks loading at the MASS to transport water to the Headwaters 142 for the frac. Water in the containment around the loading area showed conductance of 4.25 mS/cm, and was within 1-2" of the top of the berm. I noted several puddles on the pad, mostly to the west and north of the well heads, that showed conductance of 2.45-3.05 mS/cm. Rob Bowens of JKLM was on the site, and told me that the vac truck that had been working at the 117 pad would be coming to the 115 next. Violations are pending from an 8/9/18 inspection, including 78a.56, CSL 402, and SWMA 301.
2803089 Primary Facility 2018-11-14 Routine/Complete Inspection Violation(s) Noted At the time of this inspection this well was in production. There are four (4) annulus vents openly venting on this well. Annulus vent surface x intermediate (18 5/8" x 13 3/8") was noted venting 100% LEL (lower explosion limit) and 20% METHANE and annulus vent intermediate one (1) x intermediate two (2) ( 13 3/8" x 9 5/8") was noted venting 100% LEL (lower explosion limit) and 30% METHANE combustible gas at this inspection. Combustible gas was also detected at the surface x intermediate annulus (18 5/8") x 13 3/8") during previous inspections conducted on 6/27/2018 (Inspection Record #2748294) and 7/30/2018 (Inspection Record #2759044). Due to the presence of combustible gas detected at this annulus, a violation for defective cement (78a.86) has been cited. Please note that combustible gas detected at the intermediate 1 x intermediate 2 annulus( 13 3/8" x 9 5/8") indicates potential defective cement. JKLM should monitor this annulus monthly and report any significant changes to the Department as soon as pos
2813881 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the JKLM - Reese Hollow 115 well pad located in Sweden Twp, Potter Co. on 12/11/2018 @1145 hours. The weather was 25 degrees, partly sunny, 2-4 inches of snow on the ground. Winds WSW @10mph, 68% humidity. NORtech personnel were present onsite during the inspection.

The well site appears to be setup for production. 6 wells were observed onsite. The pad surface was covered by snow and ice. No issues were observed. No new violations are noted at this time. I departed the site at 1245 hours.

The following violations remain OUTSTANDING against the 1H well from inspection report #2778074 dated 9/21/2018, awaiting approval of the Final Report/Remedial Action Completion Report from the ECB. 25 Pa Code 78a.56(a) regarding Temporary Storage 35 PS § 691.402 (Potential to Pollute Waters of then Commonwealth) 35 PS § 6018.301 SWMA
