2609610 Primary Facility 2017-06-29 Routine/Partial Inspection No Violations Noted On 6/29/17 at 1400 hrs I attended a preconstruction meeting for the Headwaters 142 well site. JKLM representatives, Jeff Long and John Manuel, and J.L. Watts representative, John Watts, were present. Penn E&R developed the Erosion and Sediment (E&S) control plan and J.L Watts is the construction contractor.

We reviewed the E&S drawing. Site drainage is to Cushing Hollow, which is designated as a High Quality stream. A majority of the access road was constructed under the Headwaters 143 ESCGP. The access road for the 142 will come off the existing road and is 750 feet in length. The site will consist of a well pad (approximately 350 feet x 500 feet) and two sediment traps; Sediment Trap 1 (ST-1) and Sediment Trap 2 (ST-2) will be located beyond the southwest side of the pad. The pad will be soil cemented for stability and sumps will be installed in the south and west corners. A pipeline spur (550 feet in length) will connect the 142 well pad to the 143.

2639525 Primary Facility 2017-09-22 Routine/Complete Inspection No Violations Noted On 9/22/17 at 0934 hrs I conducted a routine inspection at the Headwaters 142 well site. It was mostly sunny and 60 degrees F. The construction contractor, J.L. Watts, was on site.

I observed the site, the Erosion and Sediment (E&S) plan and the routine E&S inspection log. New Growth Resources has finished clearing the pad and pipeline spur. The site had been grubbed and wood chip filter berms were installed along the south end of the site. Cut and fill work was ongoing to construct the site to final grade.

Well permits for the Headwaters 142 1HU - 3HU and 4V have been issued.

The last routine E&S inspection was conducted on 9/14/17. No violations are cited during this inspection. I concluded the inspection at 1054 hrs. TLT

2657847 Primary Facility 2017-11-07 Preoperation Inspection No Violations Noted A pre-spud inspection was completed at the JKLM Headwaters 142 well pad located in Ulysses Township, Potter County on November 7, 2017 at 1030 hrs. The weather was 34 degrees, light snow. Present onsite were myself Jeff long, JKLM and John Watts, JL Watts.

This inspection meets the requirements of 58 Pa. C.S. 3258 (a.1) regarding a "pre-operational inspection".

No violations are noted at this time.

I departed the site at 1200 hours.

2667520 Primary Facility 2017-12-05 Routine/Complete Inspection No Violations Noted Weather is 43 Deg. F., rainy and cloudy at time of inspection. Routine inspection of JKLM LLC, Headwaters 142 Pad on December, 5 2017.

There are total of 4 wells on location. Conductor only on all four : 2HU drilled and cemented on 11/30/17. Conductor depth is 80 ft., and the size - 26" diameter. We physically inspected the pad, site looked clean.

DEP representatives - Ulyana Trynovich and Lester Taylor. JKLM representative is not present during the inspection.

2686850 Primary Facility 2018-01-25 Drilling/Alteration No Violations Noted At the time of this inspection this well has had a conductor that is 26" x 80' set in place and a surface casing that is 18 5/8" x 639.37' set in place. The Had rig 247 and crew were doing the drilling and setting in the casings. At this time of this inspection they were waiting on cement. Chris Wiley was the company rep on location at this inspection.
2692854 Primary Facility 2018-02-06 Incident- Response to Accident or Event No Violations Noted The Department of Environmental Protection (DEP) responded to notification of a release on the JKLM Headwaters 142 well pad located in Ulysses Twp., Potter Co. Spill notification #332209 was reported to the Department by Joe Harrick, JKLM on 2/5/2018 at 1600 hours with the following details. Approximately 400-500 gallons of fluid with a conductivity of 2.5 mS/cm was released to the pad surface from a freshwater storage tank leaking approximately 1-2 gpm.

I arrived onsite @1200 hours. The HAD rig is currently onsite drilling the vertical portion of the 1HU, 2HU, 3HU and 5HM wells. The weather was 22 degrees with ~3-4” of snow on the ground. JKLM representatives Joe Harrick and Jon Manuel were present for my inspection. Pipeline construction personnel discovered the leak on the rear side of the AST (Aboveground Storage Tank) at approximately 1300 hours on the previous day. A small trench had been dug below the leak and a duck pond was installed to facilitate removal of the fluid with a vac truck. The field co

2694238 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted I arrived with Clark Smith (Environmental Cleanups Program) to follow up on the release from the AST on the site reported on 2/5/2018. I met with John Manuel and Chris Wiley of JKLM. The HAD rig onsite had finished up drilling, and the rig and equipment were expected to be cleared from the site soon. According to John, the AST contained approximately 20,000 barrels of water when the leak started at a volume of 1-2 gallons per minute. A sump was quickly dug and lined at the leak because conductance readings of 2.5 to 3 mS/cm were found in the water. The cause of the elevated conductance has not yet been determined, but John felt that the most likely explanation was that a truck carrying produced water from the Headwaters 143 pad had mistakenly come to the 142 pad instead of the frac on the 145 pad. John was unable to provide a volume of water collected from the sump, other than to say it was more than 42 gallons. 2703391 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the JKLM headwaters 142 well pad located in Ulysses Township, Potter County on March 8, 2018 at 1215 hrs. the weather was 27° and cloudy with snow squalls in the area. Approximately 2 to 4 inches of snow on the ground. The surface condition of the well pad was very muddy. Steve Mills, JKLM representative was on location.

And above ground storage tank (AST) containing what is labeled as freshwater is located centrally on the pad. Mr. Mills stated even though this tank is for freshwater use, it is constructed with a double liner. Constant truck traffic was observed onsite offloading freshwater into the AST. A temporary waterline carries the water to the AST on the 143 well pad, and from there it is pumped to the ongoing frack at the 145 well pad.

The aboveground storage tank that was reported leaking on February 5, 2018 has been dismantled and removed from the site. There are four waste containers observed on site. It is not clear if this material is dril

2711869 Primary Facility 2018-03-19 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was top holed only. The conductor casing(25") x surface casing (18 5/8") and the surface casing(18 5/8") x intermediate casing ( 13 3/8") were vented to surface. Testing for combustable gas was done on both of these annulus vents with the results being no combustable gas detected on the conductor x surface casing (25" x 18 5/8"). And 5% LEL (lower explosion limit) and 0% Methane detected on the surface casing x intermediate 18 5/8" x 13 3/8". No other combustable gas was detected on the wellhead assembly or the well area.
2715369 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection completed a routine inspection of the JKLM Headwaters 142 well pad with Sean VanFleet, DEP Env. Protection Specialist on 4/4/2018 @ 1245 hours. The weather was 38 degrees, strong wind gusts with snow squalls in the area. JKLM is preparing for the H&P 500 rig to complete drilling of the 1HU, 2HU, 3HU and 5HU wells. Pad-wide containment and rig matting is in place. A rig mat washing station was setup along the southern edge of the containment. Cleaned rig mats were stockpiled in the southeast corner. Mud tanks were observed in secondary containment in the southwest corner of containment.

Since the frack on the HW 145 has been completed, the freshwater AST has been removed from the site. Penn E&R attempted to collect confirmatory samples to meet Act 2 guidance regarding AST release #332209, but was unable to do so due to containment. JKLM stated they are unable to collect any confirmatory soil samples at the pad until July.

No new violations are noted at this time. V

2732032 Primary Facility 2018-05-17 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM – Headwaters 142 Well Pad located in Ulysses Twp., Potter Co. on 5/17/2018 @ 1145 hours. The weather was 72 degrees and sunny. Joe Huston, JKLM was present onsite for the inspection.

The H&P 500 flex rig is currently onsite drilling the 1HU well. The rig will drill 4 wells while onsite. The pad surface, containment and E&S controls were observed while onsite. All puddles located near the edge of containment on the pad surface were tested for conductivity and found to be within normal range. Fluid observed within containment was tested for field conductivity and found to be within normal range. Mr. Huston was notified that fluid appeared to be accumulating in the SW corner of the pad containment, below the STRAD tanks, due to a low spot on the pad. Mr. Huston stated he would monitor it, and remove fluid as needed to maintain adequate storage volume.

2742231 Primary Facility 2018-06-18 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was being bore hole drilled only. Projected time of finishing boring out hole ,cleaning up hole and starting to run 5 1/2" production pipe is Thursday the 21st. No issues were noted at the time of this inspection.
2756854 Primary Facility 2018-07-25 Routine/Partial/Aerial Inspection No Violations Noted The DEP conducted a routine partial inspection of the JKLM - HW 142 well pad located in Ulysses Twp, Potter Co. on 7/25/2018 @1400 hours. The weather was 74 degrees; overcast/lightrain, humidity 86%, winds SSW 6mph. JKLM has just started breaking down the rig in preparation for transport. Ron Hall, JKLM Drilling Consultant was present onsite for the inspection. The pad surface was very wet and muddy due to recent heavy rains.

2760890 Primary Facility 2018-08-06 Routine/Complete Inspection No Violations Noted At the time of this inspection the stimulation had not started on this well and the service company(Halliburton) and support crew were rigging up(preparing) to start the stimulation(hydraulic fracturing) of this well. No violations were noted at the time of this inspection.
2763634 Primary Facility 2018-08-14 Routine/Complete Inspection No Violations Noted At the time of this inspection this well has not had the stimulation process(hydraulic fracturing) started yet.
2766619 Primary Facility 2018-08-20 Routine/Complete Inspection No Violations Noted I arrived on-site and met Scott West of JKLM. Halliburton frac was active. Near the loading area for the MASS, several small puddles (1-2 gallons) were present showing elevated conductance (3.0-3.4 mS/cm). A mini excavator and vac truck were working in the area between the sitewide containment and the MASS; Terra Tokarz of Whipper Hill was guiding the excavation with a soil conductance meter. A skid steer was removing berm ramps on the back side of the pad near this area, and small amounts of water was puddling in the impressions left by the ramps; conductance readings were in the range of 9.0-9.5 mS/cm. Overall, containments appeared to be in good condition, and the elevated conductance water appeared to be related to the release from the loading area reported on 8/15/18. Outstanding violations remain for 25 PA Code 78a.56, 78a.64(a), 35 PS 691.402, and SWMA 301, and a violation is pending for 35 PS 691.401.
2768068 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was being zipper fractured (fracturing being done to more than one well at a time) with the 105-21907 5HM. 8 stages had been completed at the time of this inspection. No violations noted at this inspection.
2772944 Primary Facility 2018-08-16 Incident- Response to Accident or Event No Violations Noted The DEP conducted an INCIDENT inspection of the JKLM Headwaters 142 well pad on 8/16/2018 @1200 hours, in response to spill report #336280. An unknown quantity of fluid was released to the ground surface from the secondary containment under the AST load/offload area. Upon further investigation by JKLM personnel, it was discovered the containment was improperly constructed. The containment was missing welds, or the pieces had been improperly welded together resulting in a failure of the containment. Also present onsite for this inspection was Korey Starkey, DEP Env Trainee; Mike Jurbala, DEP Env Trainee; Sara Metz, DEP Env Trainee; Joe Harrick, JKLM EHS GM; and Scott Blauvelt, JKLM Director EHS.

At the time of this inspection, JKLM had removed the containment from the load/offload area in front of the AST. The top 2-3" of pad material beneath the containment had been removed and placed in roll-off containers onsite. 2774765 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted At the time of this inspection this wells stimulation process (Hydraulic Fracturing) had been completed. The stimulation started on 8-25-18 and was completed on 9-6-18. No violations noted at this inspection.
2783262 Primary Facility 2018-09-26 Follow-up Inspection No Violations Noted The DEP conducted a Follow-Up Inspection on the JKLM Headwaters 142 well pad located in Ulysses Twp, Potter Co. on 9/26/2018 @1200 hours. The weather was rain, 69 degrees winds NE @9mph. Personnel onsite appeared to be removing the rig matting in preparation for removing the secondary containment liner. The site was very wet due to rainfall. Multiple areas of standing water were observed on the pad surface.

The AST is still located onsite. The containment in the AST load/offload area was holding fluid with a field measured conductivity of ~300 uS/cm. Fluid was being removed from the concrete drop box in the western corner of the pad via a vac truck. An area of standing water between the AST and the western corner of the secondary containment liner had a field measured conductivity of 1.7 mS/cm. This area is being monitored for conductivity by JKLM.

JKLM plans to begin screening the pad surface and remediating the site as soon as the containment is removed. Violations of 25 Pa Code 78a.56(a), 25 Pa Code 78

2816966 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the JKLM Headwaters 142 well pad located in Ulysses Twp, Potter Co on 12/13/2018 @1130 hours. The weather was 29 degrees, overcast, 2-4" of snow on the ground, winds SSE @8mph, humidity 89%. Personnel from Hillsdale, NORtech and Well Versed were onsite during the inspection.

Onsite activities consisted of the installation of production equipment. Gas busters and a battery of tanks have been setup on secondary containment, in preparation for flow-back. The AST was still present onsite but has been drawn down. A number of waste containers were still staged onsite.

No violations are noted at this time. Violations from IR#2765037, dated 8/16/2018, remain outstanding against the 1HU well permit pending approval of the Final Report/Remedial Action Completion Report currently under review by the ECB.

2840233 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM HW 142 Well Pad located in Ulysses Twp, Potter Co. on 2/14/2019 @1130 hours. The weather was mostly sunny 30 degrees, winds SSW 10 mph, humidity 52%. The well pad and surrounding areas appear frozen and snow covered. No activity was observed onsite. The well pad appears to be set up for flowback.

Please correct any deficiencies noted in this report. No violations are noted at this time. I departed the site at 1200 hours.

2850091 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. There are four (4) annulus vents on this well that are vented to surface. These vents were tested for combustible gas with an Altair 5 x Multi Gas Meter. Results of this testing were 0% LEL (lower explosion limit) and 0% Methane on the 24" x 18 5/8" vent , 0% LEL and 0% Methane on the 18 5/8" x 13 3/8" vent , 0% LEL and 0% Methane n the 13 3/8" x 9 5/8" vent and 100% LEL and 32% Methane on the 9 5/8" x 5 1/2" vent. A gauge on the well head assembly was showing a pressure of 3,250 psi (pounds per square inch). No violations noted at this inspection.
