2737816 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was a 20" conductor only. The hole was bored (drilled) to a depth of 240' and 98' of conductor pipe was set in the hole on 6-4-2018. No water was noted during the drilling of this conductor placement. Jon Boardman E&S/Safety manager and Brad Cantwell drilling consultant were present at this inspection.
2740109 Primary Facility 2018-06-05 Routine/Complete Inspection Violation(s) Noted The Department of Environmental Protection conducted a routine inspection of the Pennsylvania General Energy – Pine Hill 2382 (Pine Hill Sylvania Pad B) located in Sylvania Twp., Potter Co. on 6/5/2018 @ 1100 hours. The weather was 65 degrees and overcast.

PGE had set the conductor for the Pine Hill Sylvania, Pad B 2740 well (API#105-21910) on the previous day. Cement was observed at the surface surrounding the conductor pipe. Two existing wells were observed onsite 105-21589 and 105-21590, that are listed as in Regulatory Inactive Status (RIS). John Boardman stated PGE plans to bring a Patterson rig to the site in July to drill the lateral portion of the 2740 well. Waste Management was onsite removing the cuttings, from the top hole portion of the well, that were stockpiled in a waste container adjacent to the well.

Onsite PGE representative Cory Dean stated they were in the process of resurfacing the pad. Stone was observed stockpiled along the northern side of the pad. Recent earth disturbance activities

2745510 Primary Facility 2018-06-25 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was conductor only. Conductor is 20" x 98' set in a 240' bore hole.
2754593 Primary Facility 2018-07-23 Routine/Complete Inspection No Violations Noted At the time of this inspection the Patterson triple 583 rig was advancing 12 1/4" pipe at an average of approximately 80' per hour. Depth of pipe was at 2402' at this inspection. No gas encountered . Jerry Nietert drilling consultant was at this inspection as was PGE company rep Zach Firduko(drilling engineer).
2761834 Primary Facility 2018-08-07 Follow-up Inspection Viol(s) Noted & Immediately Corrected The DEP conducted a Follow-Up Inspection of the PGE – Pine Hill 2382 (Pine Hill Sylvania Pad B) located in Sylvania Twp., Potter Co. on 8/7/2018 @1100 hours. The weather was 77° overcast, 71% humidity. Korey Starkey, DEP Env Trainee and Zach Fadurko, PGE were present onsite for the inspection.

The access road, well pad, containment and onsite inspection logs were inspected. The Patterson PeakeRig 83 is currently onsite drilling the Pine Hill 2740 (API#105-21910), a Utica well. At the time of the inspection the rig was at 11,036’. Mr. Fadurko stated drilling is utilizing freshwater from the onsite impoundment along with collected rainwater being stored in tanks onsite.

We departed the site at 1300 hours.

2762312 Primary Facility 2018-08-08 Routine/Complete Inspection No Violations Noted At the time of this inspection Brad Cantwell(drilling consultant) was at this inspection. The Patterson 583 Triple Rig and crew were advancing 9 5/8" production casing at an average of 9' per hour at the time of this inspection. At the time of this inspection no measurable gas was noted in drillers/ mud loggers log record. At the time of this inspection Zach Ferduko was also present. No violations were noted at the time of this inspection.
2823730 Primary Facility 2019-01-08 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the PGE - Pine Hill 2382 well pad located in Sylvania Twp, Potter Co. on 1/8/2019 @1200 hours. The weather was rain, 38 degrees, winds SSW @8mph. Ice had accumulated on the ground the previous evening.

The Pad B 2740 well appears to be shut-in after PGE flared the well the end of last year. No activity or personnel was observed onsite. The temporary water line from the impoundment appears to still be in place. Puddles on the pad surface were screened for conductivity and found to be in normal range.

No violations are noted at this time. I departed the site @1300 hours.

2825918 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted At the time of this inspection the drilling of this well was complete. Drilling was started on 7-19-18 and was finished on 9-14-18. Hydraulic fracturing (stimulation) was started on 10-15-18 with 39 total stages scheduled to be hydraulic fractured. A total of 21 stages were fractured out of the proposed 39 stages on 10-21-18. Flow back of the fractured stages was started on 11-8-18. The well as then flared from 11-12-18 until 12-9-18. There are 18 more stages proposed to finish the stimulation of this well with an undetermined date as to when the stimulation will be completed. A gauge on top of the wellhead assembly that is attached to the frac head assembly was showing 0 psi. No violations were noted at this inspection.
