2751570 Primary Facility 2018-07-02 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the JKLM headwaters 146 well pad located in you Ulysses Township, Potter County on July 2, 2018 at 1115 hrs. The weather was 85 degrees, sunny, humidity 58%, winds SSW 10 mph.

The access road to the well pad is 9700 feet long. The most of the rock-lined ditch, along the access road, appears to be installed. JL Watts was observed spreading topsoil and working on completing the access road. E.J. Breneman LP was soil cementing the pad surface. Sediment Trap 1 (ST-1) was observed off the southwest pad corner and Sediment Trap 2 (ST-2) was observed off the southeast pad corner. Sumps will be installed in the southeast and southwest corners after completion of the pad surface.

Ditches surrounding the pad surface are rough graded and draining to the installed sediment traps. Topsoil will need to be laid before the TRM matting can be installed, and the ditches completed. The topsoil stockpile adjacent to the pad was observed stabilized with mulch.

2756317 Primary Facility 2018-07-25 Preoperation Inspection No Violations Noted The DEP conducted a PREOP inspection of the JKLM – Headwaters 146 well pad located in Ulysses Twp, Potter Co. on 7/25/2018 @1230 hours. The weather was 72 degrees, light rain, Humidity 86%, winds SSW 6mph. Potter County has received approximately 6 inches of rain since Sunday, 7/22/2018. JKLM has just completed construction of the HW 146 well pad and requested a Pre-spud inspection. John Watts, JL Watts (contractor) and Jeff Long, JKLM were present onsite.

The site consists of a minor road crossing (GP-07-5329-17-001) of Cushing hollow at the base of the access road. The access road then travels ~9700 feet up to the top of the ridge where a 350’ x 500’ well pad has been constructed with two sediment traps. ST-1 is located beyond the southwest pad corner and ST-2 is off the southeast pad corner.

The permit also covers 5,560 feet of gathering pipeline which connects the HW 146 well pad to the HW 143 well pad. The ROW has been cut at this time, but not grubbed. The original plan was to HDD the pipeline under

2817664 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the JKLM Headwaters 146 well pad located in Ulysses Twp, Potter Co. on 1213/2018 @1330 hours. The weather was 29 degrees, overcast, 2-4" of snow on the ground, winds SSE@8mph, humidity 89%.

JKLM Company man Ron Burdick was present onsite for the inspection. Onsite activity consisted of the falcon rig "batch drilling" two wells onsite. The rig is currently drilling the 1st intermediate string casing of the 2HU well at a depth of 1,491'. The containment around the rig and berms appeared to be intact and functional. No staining or melted areas were observed around the perimeter of the containment. No violations are noted at this time. I departed the site at 1500 hours.

2822972 Primary Facility 2019-01-07 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was vertical drilled only. The conductor casing that is 20" x 120' was set on 10-4-18 thru 10-5-18. The surface casing that is 13 3/8" x 559' was set on 11-18-18 thru 12-9-18. The 1st intermediate that is 10 3/4" x 2621' was set on 11-25-18. The 2nd intermediate that is 7 5/8" x 8428' was set on 12-7-18. No violations were noted at this inspection.
2848263 Primary Facility 2019-03-04 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was top holed only ( vertically drilled ). Well has not been laterally drilled. The conductor that is 20" in diameter by 120' in length was drilled and set starting on 10-4-18 and completed on 10-5-18. The surface casing that is 13 3/8" diameter and 559' in length was completed on 11-20-18. The 1st intermediate that is 10 3/4" in diameter and 2,624' in length was completed on 11-27-18. The 2nd intermediate that is 7 5/8" in diameter and 8,429' in length was completed on 12-8-18. The conductor by surface casing is the only annulus vent that is vented to surface. The other annulus have vent valves attached but are not vented outside of the casing. No violations noted at this inspection.
