DEP reps on site were Steve Watson, Eric Rooney, and Gene Rickard. DEP met with Mr. John Smelko of Cabot. Selman reps on site to assist in gas readings and to split gas samples. Cabot shut in annulus at 9 am this morning. At 10:23 am a digital pressure gauge was installed on the 7 x 4" annulus. Pressure = 23.7 psi. Gauge was removed and open vent was screened for gas. Model 60 reading = >5% combustible gas, and recorded 100% LEL. Prior to sampling, fluid was drained from vent line. Both DEP and Cabot collected gas samples for isotopic analysis. After completion of sampling, Cabot re-connected vent line and locked vent open. Cabot is planning on moving in a workover rig this week to service well.

2509181 Primary Facility 2016-08-04 Complaint Inspection No Violations Noted At time of inspection, arrived on site at 12:35p.m. pertaining to a complaint called into the department about a backhoe and a dumpster being hauled onto the site and unknown activity. There was no activity on the site at time of inspection, there was no backhoe on site but there were two role off containers on site. The marks on the entrance road indicated that the containers have recently been dropped off.

I left the site at 12:45p.m.

2528794 Primary Facility 2016-10-11 Site Restoration No Violations Noted Inspection of the Costello J P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

Onsite to perform routine insp of the Costello 1V well, the only well drilled on this pad. Most recent remedial activities were completed on this well back in Sept. and Oct. of 2014. This remedial work included; - Removal of ~ 1776' of 4 1/2" csg. and milling the entire 7" Intermediate casing string, approx. 1500', out of wellbore. Following this work wellhead was connected to Total Flow Control Unit (TFCU) to monitor any venting that may be occurring. At time of inspection reading taken off the TFCU showed no measureable vent flow detected ¿Static¿, with a cumulative reading of 12.75 cf/d. (See attached pic# 2). Vent line from TFCU was ran to a small poly vent tank. A gas check was performed in the vent tank lid opening showed, 58% LEL, 2% CH4. A gas check performed ~ 3¿ away from vent tank at ~ 5¿ above ground level showed 0% LEL, 0% CH4. No indications of recent activities were observed at site and no further concerns were noted. No
