At time of inspection, arrived on site at 11:25 a.m., the cellar was full of gravel and dirt with no gas detected at the perimeter of the cellar, the (9x4) vent pipe was set up with a Tayler plug and a 2000psi pressure gauge that was reading about 30psi, the 4.5 production casing was set up with a Tayler plug. There were no lines going to or from the well. There was one other well on site (115-20379 2H) that was in production. I took a walk around the location and everything looked OK. There were no obvious concerns noticed at time of inspection. I left the site at 12:00 p.m.

At time of inspection, arrived on site at 9:10 a.m. to verify the pressures, I met with well tender for SWN Tyler Rowe. Tyler used a Crystal pressure gauge to check the pressures on the annuli’s that were set up with a Tayler plug. (9x4) annulus had 60.7psi 4½" production casing had 1414psi Production tubing had 1485psi There were no obvious concerns noticed at time of inspection. I left the site at 9:35 a.m.

The vent pipe was open and set up with a Taylor plug to check for pressure and a 2000psi pressure gauge with 0psi on it. There were no lines going to or from the well, the well was granted inactive status on 6/24/2016. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:40a.m.

There are two wells associated with this site: 1V 115-20099 (inactive as of 6-24-16) 2H 115-20379

This site had excellent vegetation, even visible through snow cover. There were woody plants established on the fill and cut slopes and the grasses were well established all over the site as well. There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note.

The well was granted inactive status on 6/24/16. There were no lines going to or from the well. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x4) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 2000psi pressure gauge with about 60psi on it. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

Met with Cody Stanton (Production Foreman) for SWN. The well was granted regulatory inactive status on 6/24/16. There were no lines going to or from the well. The cellar was full of gravel with water on top and there was no bubbling noticed. Cody used a Crystal pressure gauge to get the pressures for the (9x4) annulus, production casing and tubing. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
