At time of inspection, arrived on site at 11:25 a.m., the cellar was full of gravel and dirt with no gas detected at the perimeter of the cellar, the (9x4) vent pipe was set up with a Tayler plug and a 2000psi pressure gauge that was reading about 30psi, the 4.5 production casing was set up with a Tayler plug. There were no lines going to or from the well. There was one other well on site (115-20379 2H) that was in production. I took a walk around the location and everything looked OK. There were no obvious concerns noticed at time of inspection. I left the site at 12:00 p.m. |

Tyler used a Crystal pressure gauge to check the pressures on the annuli’s that were set up with a Tayler plug. (9x4) annulus had 60.7psi 4½" production casing had 1414psi Production tubing had 1485psi There were no obvious concerns noticed at time of inspection. I left the site at 9:35 a.m. |

There are two wells associated with this site: 1V 115-20099 (inactive as of 6-24-16) 2H 115-20379 This site had excellent vegetation, even visible through snow cover. There were woody plants established on the fill and cut slopes and the grasses were well established all over the site as well. There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note. There were no violations.
