2501065 Primary Facility 2016-07-13 Routine/Complete Inspection Pending At time of inspection, arrived on site at 10:00a.m., the cellar was full of gravel, the (9x5) vent pipe was set up with a Taylor plug to check for pressure. There were no lines going to or from the well and there were no pressure gauges on the well.

Gas was detected at the perimeter of the cellar at 81% LEL and 5% methane with an Altair 5X gas detector that was calibrated on 7/12/16. I contacted SWN and talked to Afton Sterling, I told her of the gas detected and suggested the gravel be removed so it could be determined where the gas was coming from. I also asked that she lets the department know when this would take place so the department could be on site. I took a walk around the location and everything looked OK. I left the site at 10:20a.m.

2503000 Primary Facility 2016-07-20 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 14:00p.m. for a follow up inspection for the inspection conducted on 7/13/16 where gas was detected in the perimeter of the cellar. I met with Mike Deming of SWN, Tom Messing and Shauna Erway of Leidos. The cellar was empty, there was water between the (16x13) annulus with no bubbling noticed, there was cement between the (13x9) annulus. I used an Altair 5X gas detector that was calibrated on 7/12/16 and Tom used a Gas Explorer by Bascom Turner to check for gas. I detected gas at one spot that peaked at 16% LEL 0% methane then depleted to 0%. Tom was getting a reading of 40ppm in the cellar; it takes 500ppm to make 1%LEL. It was determined that the reading I got on 7/13/16 was gas trapped in the dirt and gravel. Mike said SWN will keep monitoring the well and if they see any changes or bubbling, they will contact the department.

I left the site at 15:30p.m.

The site is surrounded by active agriculture with no issues. The pad surface is level and mostly vegetated through the stone.

There were no lines going to or from the well and there were no production units or tanks on the well site. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:55p.m.
