1912904 | Primary Facility | 2010-09-13 | Follow-up Inspection | No Violations Noted | Wellhead exposed. Cellar pumped down. No bubbling observed. Venting 9 5/8" x 5 1/2" to reserve pit. Still producing low volume of gas. Opened vent on 20" x 13 3/8" annulus - no gas detected. |
1923999 | Primary Facility | 2010-10-13 | Drilling/Alteration | Outstanding Violations - No Viols Req'd | Plan to do squeeze on backside of 5 1/2" . Gas is still present coming to surface between 9 5/8" and 13 3/8". 5 1/2" vented to reserve pit. UDI Rig on site. Pressure gauge will be installed on 5 1/2" annulus to monitor pressure. DEP will follow up on status of work. |
1924106 | Primary Facility | 2010-10-19 | Follow-up Inspection | No Violations Noted | Squuezed 60 sks 15.6 lb/gal on annulus of 5 1/2". At 4 lbs pressure was @ 59 psi. Able to pump another 3 1/2" bbls, and then psi spiked to 2500psi. RD halliburton and moved off location. Pressure left on well untill 12:)) pm on 17th. Released pressure then closed back in. At 800 am the following day psi at 0. Based on this, appears that problem has been fixed and gas migration through casing/cement has been corrected. At this time, no outstanding violations. Planning on fraccing well later on in week. |
1934736 | Primary Facility | 2010-12-11 | Routine/Complete Inspection | No Violations Noted | The pit is overfilled and needs to be lowered about 2 feet before solidification and encapsulation can begin. Stone Energy proposes creating a smaller temporary pit to place the excess cuttings until landfill approval is obtained. I conducted an inspection of the current pit conditions. The pit itself is satisfactory. There is no residential water supply within 200 feet of the pit. Wetlands are in the area but appear to be located at a distance greater than 100 feet of the pit. Stone Energy will be submitting OG 71 requests for the solidification and a current plan which includes the wetland delineations for confirmation. |
1952095 | Primary Facility | 2011-02-18 | Routine/Complete Inspection | No Violations Noted | On site w DEP WQS, Matt Shope, to conduct an inspection of the pit. Currently, there is no activity at this site. I observed that the pit has been completely filled in. Company Man, Dan Williams informed me that pit was encapsualted and backfilled between 2-11 and 2-13-11. The Drill Cuttings were solidified the week of 2-7-11. There were no violations observed during the inspection. Please feel free to contact me if there are any questions at 570-346-5533 or jsassi@state.pa.us. |
1965431 | Primary Facility | 2011-04-06 | Routine/Complete Inspection | No Violations Noted | Routine inspection. No activity on site. Well is shut in. When standing near well head can hear gas venting. Could not determine source. Temporary fencing in place around well heads. Freshwater impoundment on site. Fence maintained around impoundment. Freeboard @5'. Liner appears to be in good condition. DEP wil follow up with operator on status of venting at well head. |

2312492 | Primary Facility | 2014-10-10 | Incident- Response to Accident or Event | Pending | Residual producation fluid spilled to surface of pad at fluid transfer hose connection. Transfer hose connection point was constructed outside containment structure for production fluid tanks with minimal secondary containment. Production Tanks collect fluid for all three producing wells at centralized location on upper Loomis Pad. Excavation and confirmatory sampling analysis has been planned to be conducted through Leidos Environmental. |
2314756 | Primary Facility | 2014-10-10 | Incident- Response to Accident or Event | No Violations Noted | Upon completion of the inspection at the lower Loomis well pad, we proceeded to upper Loomis well pad to investigate an area of staining of the pad surface, mentioned by Mr. Gregory during the Loomis lower pad spill investigation. I observed an area of staining on the pad surface in a relatively close proximity to the production fluid tanks containment system. The vacuum truck hose connection port utilized to offload production fluid from the tanks is housed outside of the production fluid tanks containment on a slab of concrete on the surface of the pad. The connection port is minimally contained. It is believed that as hoses from vacuum trucks are disconnected upon offloading production from the tanks, fluid leaked to the surface of the pad. Mr. Gregory mixed distilled water with pad surface soil to obtain a conductivity reading in where the staining was observed. |
2327565 | Primary Facility | 2014-12-09 | Routine/Complete Inspection | No Violations Noted | Remarks: The Department Arrived on site at approximately 13:30 on December 09, 2014 at the Loomis Well Pad (ESX10-115-0010) to conduct an inspection of the ongoing contaminated material removal. The SWN employees on site were Tracey Gregory and Tom Chitwood. Renee Andrews of Leidos was on site as well. |

2445790 | Primary Facility | 2016-01-29 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 13:10 p.m., the cellar was empty, the (13x9) vent pipe was open and set up with a Taylor plug, the (9x5) vent pipe was open with about 200psi on it, the production casing was set up with a Taylor plug, there was about 400psi on the production line. I took a walk around the location and everything looked OK.
I left the site at 13:40 p.m. |
2823001 | Primary Facility | 2019-01-07 | Routine/Complete Inspection | No Violations Noted | I performed a RTNC inspection of the Loomis well pad on January 7, 2019. The weather was overcast and 20 degrees F. There was no precipitation occurring.
The access road was long, passing two separate well pads. The first well pad was small and compact. The landowner was cutting firewood at the edge of the pad, with no adverse erosional effects. The access road and pad surface were both solid and stable. The cut and fill slopes of this first well pad had good vegetative cover and there were no erosional problems noted. The access road wound its way through a second gate to a scond well pad. The well pad contained the Loomis wells as well as an impoundment. The access road and pad surface were both well constructed and stable. The impoundment had at least 3 feet of freeboard. The fence surrounding the impoundment was intact and had a life preserver and rescue ring on the surrounding fence. The slopes surrounding the upper Loomis pad were well vegetated and stable. There did not appear to be an |
