2440619 Primary Facility 2015-09-07 Routine/Complete Inspection No Violations Noted Inspection in conjunction with the notice of termination inspection of the Vandermark Pipeline. No issued observed at the time of the inspection.
2486436 Primary Facility 2016-05-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:55 p.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 665psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:20 p.m.

2641287 Primary Facility 2017-09-28 Site Restoration No Violations Noted The Vandermark well site was inspected by me for Notice of Termination (NOT) on September 26, 2017. The site was in very good shape, with no erosional issues occurring at the time of inspection. All slopes were well vegetated, over the 70% required perennial growth and all surfaces were solid and stable. There were two corners of the cut slope which appear to have been repaired with no. 4 stone. These areas were in good shape and appear to have been repaired some time ago. The pad surface was good as well with no apparent ponding issues. The signage in front of the pad was legible and overall the site appeared stable.

There were no violations noted at the time of inspection.

I recommend APPROVAL of this NOT.
