2427533 Primary Facility 2015-11-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35 a.m., the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug, there was no gas detected at the perimeter of the cellar, there was 375psi on the production casing and 356psi on the production line. I took a walk around the location and everything looked OK.

2522741 Primary Facility 2016-09-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:40p.m., the cellar was full of gravel and water with no bubbling noticed at the perimeter of the cellar. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 195psi on the production casing and 162psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 15:15p.m.

2523761 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted The Department was on site to inspect Line 426 in preparation for NOT inspection. While on site, the Department conducted an inspection of the Coyle Well Pad, ESX12-115-0056. The wells associated with this site are as follows:

The site was stable, with no erosional issues at this time, and the vegetation on site seemed healthy and of sufficient quantity to prevent issues from arising.

2617070 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:00p.m.

The cellar was full of gravel with water setting on the top with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 284psi on the production casing and 252psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:40p.m.

2625102 Primary Facility 2017-08-11 Routine/Complete Inspection No Violations Noted The Department arrived on site at 10:50 AM to conduct a Routine Complete Inspection. We reviewed the E&S Inspection Log to find that the most recent inspection was conducted on 8/7/17 with the note "rills monitor". The Department departed the site at 11:27 AM.
2720587 Primary Facility 2018-04-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was shut in at the GPU. The cellar was full of gravel with ice on top and was unable to check for gas at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
