2521878 Primary Facility 2016-09-12 Routine/Complete Inspection No Violations Noted I met with Tom Fooshee, site Company Man of SWN shortly upon arrival at the site. Falcon Drilling Rig #25 is at the location currently in the process of drilling the Conklin East 5H well. The most recent drilling resumption notice was submitted by SWN to the Department on 9/12/16. The comments in the notice indicated that Falcon 25 will be skidding to the Conklin East 5H on 9/9/16 at 5AM. Mr. Fooshee stated that the surface casing on the 5H well was cemented at (618) feet yesterday. The current depth of the well is (628) feet. The cement has cured for (8) hours and the rig crew is in the process of Formation Integrity Testing. The 2H and 6H well is in production. Falcon 25 has drilled the 1H and 4H well to kickoff point. The 3H well will be the last well drilled to kickoff point according to Mr. Fooshee. Eventually, either SDC Drilling Rig #44 or 45 will drill the 1H, 3H, 4H and 5H from kickoff point to total depth.

SWN submitted an alternative waste management practice (OG-71) application to the

2524899 Primary Facility 2016-09-30 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:05 in response to an estimated (8-10) gallon spill of oil based mud. I met with Dan Hesserdorfer and Jonathan "Woody" Woodburn, both site company men of SWN, Danica Westgard, Drilling Engineer in Training and Mike Deming, Senior Regulatory Analyst also of SWN. We discussed the specifics of the spill incident in the company man trailer prior to viewing the area of impact and remediation process where the spill occurred. SDC Drilling Rig #45 is currently in the process of drilling the Conklin East 5H well at a depth of (10,820) feet. Re-entry of the well at kickoff point commenced at (5,243) feet. The well will be drilled to total depth at (11,991) feet). The Conklin East 2H and 6H wells are in production. SWN personnel stated the following information in reference to the spill. "The spill actually occurred at on 9-29-16 around 12:30 AM during a transfer of mud from a frac tank to the mud tanks next to the drilling rig. A valve was left open a frac tank forcing the oil based
2553051 Primary Facility 2017-01-18 Administrative/File Review No Violations Noted On 1/10/17 the Department submitted a Statewide Health Final Report Approval to SWN in reference to a spill of oil based mud that occurred at the site on 9/29/16. The report states the following information. "The Department of Environmental Protection (DEP) received on December 22, 2016 and reviewed the Document titled " Site Characterization And Final Report For Site Remediated to Statewide Health Standard, " (report) for the Range Unit 36- Conklin East Pad. Approximately one barrel of diesel based drilling fluid was released to secondary containment, breached the containment and impacted underlying well pad soils. The report was prepared by Resource Environmental Management, Inc. and submitted to DEP in accordance with the Land Recycling and Environmental Remediation Standards Act (Act 2) and constitutes a final report as defined in Chapter 3 of Act 2. The DEP hereby approves this final report for the substances identified and remediated to an Act 2 standard within the site specified. Chapter 5, Sect
2562358 Primary Facility 2017-01-30 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:30. I met with Eugene Mancuso, Production Foreman of SWN. The most recent stimulation notice for the Conklin East 1H well was submitted to the Department from SWN was on 11/23/16. The notice indicated that stimulation would commence on 11/25/16 at 6:00. Mr. Mancuso stated the following. "The frac job has been done for a few weeks now. We are setting up production equipment and welding pipeline. Flowback of the wells will start in about a month or so. The majority of the frac tanks and containment might be removed prior to flowback. There are 6 wells here and 2 are in production."

The mailbox at the site entrance contains a Prepardeness, Prevention and Contingency Plan dated May of 2012, a site specific Emergecy Response Plan, Erosion & Sedimentation Control Plan, a site specific Spill Prevention Control and Countermeasure checklist and a Weekly E&S Inspection Checklist and Site Inspection Log. 2580405 Primary Facility 2017-04-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:50a.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 783psi on the production casing and 659psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:45a.m.

2591948 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted Inspection conducted on May 4, 2017. A crew is actively working on restoring ditches and cleaning out cross culverts. It appears as if all 6 wells have been placed into production. The East 3H, East 1H, East 4H, East 5H, East 6H, and the East 2H. Overall, site stable. No violations noted. Standing water on west side of well heads had a conductivity of 1000 micro Siemens. Photo 6 shows what appears to be land owner damage off access road.

There was containment around the four newest wells drilled and an empty flow back tank on the containment, the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
