Well permits that have been issued for this pad are the 3H (115-21348), 4H (115-21349), and the 5H (115-21350). All perimeter BMP’s in place and include red lined additions. The temporary Sediment Trap #1 has been installed to include the 11’ spill way. Pad construction has not been started yet. No violations noted.
2236813 Primary Facility 2014-01-13 Incident- Response to Accident or Event Violation(s) Noted Arrived at the site at 12:30 in response to an estimated spill of (90) barrels of flowback fluid. Cabot contacted the Department at 12:13 AM on 1/12/14 to report the spill incident. Gene Trowbridge, Water Pollution Biologist of PADEP NERO ERT responded to the incident at 1 AM on 1/12/14. Steve Watson, EHS specialist of Cabot accompanied Mr. Trowbridge during the inspection. The DEP Emergency Response Field Inspection report states that the wells on the Pad had been in production for a couple of weeks and a frac tank collecting flowback water adjacent to the wells ruptured spilling (90) barrels of flowback water to the Pad surface. The report states a Somerset employee was in the process of checking the fluid level in the tank and he used a cell phone as a flashlight, which ignited the vapors in the tank. This resulted in an explosion and ruptured the tank. The wells were shut in immediately following the incident and vacuum trucks were dispatched to the site to collect the spilled fluid.

There was 855psi on the production casing and 597psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 9:40a.m.

2729178 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection on the Reynolds R pad. The wells associated with this site are as follows: 115-21348, 115-21349, 115-21350. The wells were properly labeled.

The pad surface and access road were solid and stable.

The cut slope showed evidence of previous repairs but was otherwise well vegetated. There did not appear to be any areas of instability.

The fill slope had a rill forming midway across the face. (See photo) There was apparently a drain from the pad surface daylighted near the South side of the pad. Downhill from this drain was a somewhat swampy area that appeared to be eroding. At this point the erosion from the small rill and pad drain is minimal but it is recommended this should be addressed promptly. (See photos)

There was an infiltration basin that had an overflow rock apron over plastic. The rock has been pushed off the plastic by the force of runoff. (See photo) It is recommended this be repaired promptly.

There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
