I took a walk around the location and everything looked OK.
2537816 Primary Facility 2016-11-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15a.m., the cellar was about a quarter full of water with no bubbling noticed. The (13x9) vent pipe was open and venting into a small poly tank, the (9x5) vent pipe was open and venting into a small poly tank. There was 517psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:40a.m.

2575381 Primary Facility 2017-03-24 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Acker Well Site. The E&S Inspection Log indicated that Sam Wiernusz conducted the most recent inspection on 3/13/17.

Operator, please be advised that 102.4(b)(5)(x) states "A maintenance program which provides for the operation and maintenance of BMPs and the inspection of BMPs on a weekly basis and after each stormwater event, including the repair or replacement of BMPs to ensure effective and efficient operation. The program must provide for completion of a written report documenting each inspection and all BMP repair, or replacement and maintenance activities."

2624157 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 487psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:30p.m.

2670127 Primary Facility 2017-12-12 Routine/Complete Inspection No Violations Noted I inspected the Acker well site on December 12, 2017. The access road was excellent. It was stable and the front part was paved and had no potholes. The access road runs next to a valve set. This was camouflaged from homeowner view by evergreens, which were healthy and lush. The site itself was small and well maintained. The pad surface was solid and stable and the cut and fill slopes were gentle and well vegetated. There were no erosional issues on site at the time of inspection.

There were no violations.

2730718 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5.5' below grade in the cellar and there was no bubbling noticed, there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes are connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2832301 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was ice at the bottom of the cellar and there was no gas detected. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
