The secondary containment system for the light tower in the backyard was not properly secured. . A request for approval of an Alternative Waste Management Practice (OG-71) application needs to be submitted to the Department for the solids control process. The request for an OG-71 approval encomapsses both the use of centrifuges for seperation of solid and liquid material in the drill cuttings during oil and water based mud drilling and the injection of the chemicals mentioned above used for flocculation during water based mud drilling. |

Brody Webster, Steve Watson and Bob Raup, Environmental Health and Safety Specialists of Cabot Oil and Gas were at the site during the inspection. There was no activity on the tank pad. Flowback procedures for all the wells at the site have recently concluded. Erick Flowback Services was subcontracted by Cabot to conduct flowback procedures. Mr. Webster reported the spill to the Department at 12:18 on 11/10/14. He also reported an additional flowback fluid spill of less than (1) barrel, (42) gallons in volume discovered on the tank pad on 11/10/14. He explained that the flowback fluid spill where (5) barrels impacted the tank pad surface, occurred due to a butterfly valve left open on a frac tank during the transfer of flowback fluid from the well pad to the tank pad. |

-Onsite in follow-up to initial inspection performed on 12/15/17 which detected 100% LEL and 100% CH4 at both shared annuli vent tanks.
