2284108 Primary Facility 2014-07-03 Preoperation Inspection No Violations Noted The Department was on site at the Talisman Energy Choconut Valley Farms Well Site to conduct a Preoperational Inspection. The Well Permit issued for this pad is the Corey 1H 115-21722. The perimeter BMP's were in place and the pad was less than 40% complete. There were large wood chip piles on site which were in the process of being removed during inspection. Rock sumps for the culvert piping across the access road were in the process of being completed. Due to recent rain storm activity, progress on site was not as advanced as anticipated, however a post rain event inspection was recently completed. The most recent inspection activity was 7/2/2014. 2335305 Primary Facility 2014-12-31 Routine/Complete Inspection No Violations Noted The Department was on site at the Talisman Energy Corey Well Site to conduct a Routine Complete Inspection. The Well Permits issued for this pad are the Corey 1H (115-21722, 115-21723). The site was stable and there were no erosional issues at this time. The most recent inspection activity was 12/31/2014. 2375390 Primary Facility 2015-06-01 Routine/Complete Inspection No Violations Noted The Department was on site at the Corey Well Pad to do a routine complete inspection of the erosion and sediment controls. The site was not well vegetated, and recently had a small slip near the corner of the pad on the cut slope. The site surface is hydroseeded with a tackifying agent, and the slip had been recently repaired with rip rap so it is now stable. There was no evidence of any sediment leaving the site, and there is a contractor scheduled to scarify and reseed all poorly vegetated areas.

2491430 Primary Facility 2016-06-08 Routine/Partial Inspection No Violations Noted The Department was on site to conduct a Routine Partial Inspection at the Corey Well Site located in Choconut Township, Susquehanna County. At the time of the inspection, the E&S Plan was on site and the E&S inspection log indicated that the most recent inspection was conducted on 5/29/16.

The Department noted that there are areas along the slopes on the access road that do not appear to meet temporary stabilization requirements. The Department requests that the operator deploy a method of temporary stabilization that is consistent with your E&S Plan within 3 days of receipt of this report.

In the area where the culvert (STA 18+50,Stream Crossing of the UNT to Choconut Creek -WWF/MF) is, there are rills running down the access road. Sediment laden water is running down these rills, across the road, down the rip rap fill slope, and into the UNT to Choconut Creek. The creek is being impacted with sediment laden water but is running clear upstream from this interference. This is a violation of the Penn

2498596 Primary Facility 2016-07-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:55a.m., the cellar had water at the bottom with no bubbling noticed, the (20x13) vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 16% methane with no measurable flow. There was 1420psi on the production casing and 1149psi on the production tubing. The production line was connected to a separator and the bottom line was going to a 500bbl tank that was on containment and the valve was closed. I took a walk around the location and everything looked OK.

I left the site at 10:35a.m.

2526660 Primary Facility 2016-09-26 Follow-up Inspection No Violations Noted The Department was on site to conduct a Follow-Up Inspection of the Corey Well Site access road in the vicinity of the areas of concern documented in the 6/8/2016 inspection. At the time of the inspection, the Department noted that the UNT to Choconut Creek was running clear. The area has been stabilized, and the side of the road has been built up and the material seems to be compacted. A filter sock sediment trap is located at the base of the road alongside the stream as an extra layer of protection. At the top of the slope (on the access road) “rip rap” is spray painted with two lines continuing toward the FSST. New filter sock has also been added and several water bars have been installed on the access road directing the flow of water to either side. The culvert outlet adjacent to the pipeline is still full of sediment and should be maintained. The pipeline interface also appears to be better vegetated than in previous inspections. 2588470 Primary Facility 2017-04-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:15p.m.

The cellar was about a quarter full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) annulus vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) annulus vent pipe was open and venting into the atmosphere with gas detected at 100% LEL and peaked at 38% methane with no measurable flow. There was 823psi on the production casing and 465psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:50p.m.

2628472 Primary Facility 2017-08-23 Routine/Complete Inspection No Violations Noted I inspected the Corey well site on August 1, 2017 and found the site to be stable and well vegetated at the time of inspection. The cut and fill slopes had strong vegetation and there did not appear to be any erosional issues or slides at the time of inspection. The pad surface was solid and stable, and the access road, shared with the Choconut Valley Farms well site, was acceptable. It had some washing visible however all waters off site were clear and there didnt appear to be any sediment off site.

There were no violations noted.

2696026 Primary Facility 2018-02-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2.5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. There was water 1.5' below grade in the cellar and there was no bubbling noticed. The (20x13), (13x9) and (9x5) annuli were venting to the atmosphere. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
