2520127 Primary Facility 2016-09-15 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:35a.m. for a new spud inspection, Well Service Group was on site drilling the hole and running the conductor pipe. The cellar was in place but the hole for the conductor pipe wasn’t drilled yet, they were waiting on a different drill bit. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. I took a walk around the location and noticed the tanks and production units were taken off location and everything looked OK.

Depth at time of inspection was 6'. Total depth of the conductor pipe will be 45'. I left the site at 12:25a.m.

2520270 Primary Facility 2016-09-01 Routine/Partial Inspection No Violations Noted
2522744 Primary Facility 2016-09-26 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 10:20a.m., I met with company man Kevin Heathcock, Falcon Rig 25 was on site and drilling the hole for the top section of the well. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 3403'. Projected TD for the top hole section is 5228' I left the site at 11:30a.m.

2537814 Primary Facility 2016-11-15 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:20p.m., I met with company man Doug Lockwood, SDC Rig 44 was on site and drilling the production hole for the lateral section of the well. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 17313'. Projected TD for the well is 18488' I left the site at 14:40p.m.

2552777 Primary Facility 2017-01-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:40a.m., I met with company man Joe Burchett. U S Well Services was on site to frack the 4H and 5H wells. When I arrived on site they were pulling the perforation tool out of the hole on the 4H well, they were setting up to frack stage 41 out of 46 stages on the 4H. The 5H well hasn’t been started yet but will have 57 stages. I took a walk around the location and there were no obvious concerns noticed. The water is being hauled to 500bbl tanks set up on the Herman pad and pumped to the Webster pad through the gas line. The tanks were on containment and the containment appeared to be in good condition.

I left the site at 10:50a.m.

2556435 Primary Facility 2017-01-27 Incident- Response to Accident or Event No Violations Noted I traveled to the WR 73- Webster Well Site in response to a complaint of mud being tracked from the SWN Webster access road onto Hawk Road. At the time of my arrival, Jon Sass (DEP-EPS) and I walked along Hawk Road and found that sediment was leaving the well site and flowing down the roadside ditch heading east in said roadside ditch along Hawk Road. Mr. Sassi found that a cross culvert on Hawk Road was carrying the sediment from the north side of the road out to the southern side of the road and depositing it in a stream channel. It appears that the stream channel is the headwaters of DuBois Creek (CWF, MF). The roadside ditch was nearly full of sediment east of the well site. Any water moving through the ditch in this area was heavily sediment laden. I found that the roadside ditch west of the well site was running clear and nearly sediment free aside from the 2’-3’ from the access road where it appeared that some sediment found its way into the ditch.

2561730 Primary Facility 2017-01-31 Follow-up Inspection No Violations Noted The Department was on site to meet with Seth Tratthen and Steve Moyer (SWN) to follow up on the Department’s Notice of Violation dated 1/27/2017. At the time of my arrival, I walked along the access road. The site was snow covered and it was currently snowing during the inspection. I moved some small areas of snow aside to view the status of the access road after SWN reported to have repaired it. I noted that the stone on the surface of the access road does not appear to be the proper size as denoted in the Erosion and Sediment Pollution Control Program Manual.

Mr. Moyer explained that they were advised to “choke” the access road by placing smaller sized material in an effort to solidify the road. We then walked to the base of the access road and viewed the Hawk Road Roadside ditch, which although snow covered, was found to have been reworked and the stone that they used was visible through the snow cover. The culvert inlet downslope was found to have clear water running through it. The outlet also

2569561 Primary Facility 2017-02-08 Incident- Response to Accident or Event No Violations Noted The Department (Jon Sassi and I) arrived on site to conduct an Incident Inspection in response to Mr. Demming's report. At the time of the inspection Consolidated Well Services was drilling out frac plugs. We met with Justin Johnson (Construction Supervisor-John Miller) at the time of the inspection. Mike Demming later arrived on site and both gentlemen joined us during our inspection. We walked along the access road and moved stone around in areas that appeared to have residual sheen. We followed the residual sheen upslope to an area on the access road that appeared to be the point of impact. Mr. Demming field screened the areas and got readings between 0.7-112 PPM on his PID. Ed Blystone (Well Site Supervisor) met with us during the screening.

We then walked down the access road where Mr. Demming explained that an approximate 1 quart of diesel fuel was recovered by the vac truck at the Hawk Road roadside ditch. They also removed stone and some material beneath the stone. When we inspected the area,

2573027 Primary Facility 2017-03-09 Follow-up Inspection No Violations Noted We met with Mike Deming (Senior Regulatory Analyst-SWN) and Adam Brace (Production Foreman-SWN) during this inspection. During the inspection, the Department was approached by a complainant who voiced their issues concerning the condition of Hawk Road. They explained that the increased truck traffic due to activity at the well site has pushed material into the roadside ditch (because the trucks are driving at the edge of the roadway)causing sediment laden water to enter the stream.

They requested that the Department have SWN repair Hawk Road and install catchment basins at the culvert outlets along the roadway. The complainant explained that when Alta was operating the site, that a similar complaint was made due to sediment entering a pond in the vicinity. They showed us an area where they said rock was added to the roadside ditch in response to that issue.

2586479 Primary Facility 2017-04-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:10a.m.

The well was recently completed and is in production and producing through a separator. The well tag was on the well and correct. There was containment around the well and under two 500bbl flow back tanks, the containment appeared to be in good condition. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 638psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:25p.m.

2586481 Primary Facility 2017-04-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:10a.m.

The well was recently completed and is in production and producing through a separator. The well tag was on the well and correct. There was containment around the well and under two 500bbl flow back tanks, the containment appeared to be in good condition. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. 2668910 Primary Facility 2017-12-08 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection of the Webster well pad on December 8, 2017. The pad was stable, as well as the access road. The cut and fill slopes had vegetation where it was able to grow, but it was fairly sparse due to the very rocky nature of the soil. The wells onsite were labeled. The containment on site was not full and there did not appear to be any erosional problems at the time of inspection.

There were no violations noted.

2697766 Primary Facility 2018-02-15 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2794461 Primary Facility 2018-10-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1.5' below grade in the cellar and there was no bubbling noticed. There was a role off container on containment (see photo) that had dirt in it that was scraped up with foaming agent, the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
