2557741 Primary Facility 2017-01-03 Preoperation Inspection No Violations Noted The Department was on site at the Bolles South Well Site to conduct a Preoperational Inspection. The Department met on site with Rich Gulyas and Seth Tratthen (SWN). At the time of the inspection, the site was covered in snow.

S. Moyer conducted a Post Weather Inspection on 12/7/16 and he documented that no attention was needed.

The Department walked the site with the above mentioned individuals and determined that it appears as though the perimeter BMP's have been installed.

2585090 Primary Facility 2017-04-19 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:20a.m. for a new spud inspection.

WSG Rig #1 was on site and drilled the hole and ran the conductor pipe. I met with company man Robert Gray. The conductor pipe was set at 57' and cemented to the surface. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:00p.m.

2600914 Primary Facility 2017-06-02 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:20a.m.

I met with company man Andrew Griffin. SDC Rig 45 was on site and drill the lateral section of the well. Top hole section of the well was drilled and completed by WSG Rig#1on 4/23/17. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 15,238'. Projected TD for the well is 17,673'. I left the site at 12:15p.m.

2613965 Primary Facility 2017-07-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:10p.m.

I met with company men Aron Lash and Chris Henry. Halliburton was on site to stimulate the 1H,2H, and 3H wells. They completed stage 17 out of 56 stages on the 2H and were waiting for some packing repairs before starting stage 18. The 1H had 40 stages completed out of 42 stages. The 3H hasn’t been started yet but will have 88 stages when completed. The water was being hauled to the site from the Zeffer production water impoundment and Odell fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:40p.m.

2653310 Primary Facility 2017-10-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. There were 22 500bbl frac tanks on site and on containment. The tanks were empty and the containment appeared to be in good condition. I took a walk around the location and there were obvious concerns noticed. There were no violation noted.

2755845 Primary Facility 2018-07-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2856955 Primary Facility 2019-03-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. There was an empty 500bbl tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
