2593740 Primary Facility 2017-05-16 Routine/Complete Inspection No Violations Noted On Monday, May 15, 2017, I conducted a routine-complete inspection at the Bercosky Well Site located in West Bethlehem Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which is secured with a locked gate. The access road and well pad are in good condition and the site is stabilized with vegetative growth. The operator should continue their periodic monitoring program and address any deficiencies, signs of accelerated erosion and/or sedimentation upon identification.

The earthen berm containment structure appeared to be in good condition and is partially vegetated. No signs of any previous release were apparent. The gas wells appear to be in good working order and are labelled accordingly.

No violations were identified during the inspection.
