2399855 Primary Facility 2015-07-31 Site Restoration No Violations Noted On 7/31/15, I inspected Bercosky Unit site in response to an NOT request and made the following observations:
2593591 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted On Monday, May 15, 2017, I completed a routine-complete inspection of the Berkosky Well Site located in West Bethlehem Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which was secured with a locked gate. All required documentation was present on-site. The access road and the well pad was in good condition and the site has been stabilized with vegetative growth. The operator should continue their periodic monitoring program and address any deficiencies, signs of accelerated erosion and/or sedimentation upon identification.

The secondary containment structure around the production tanks appears to be in good condition and is functioning as intended. The gas wells appear to be in good working order and have been labelled.

No violations were identified during the inspection.
