2630099 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the John Mocek well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained, no leaks or fluids around well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

Six 500 bbl tanks installed within stainless steel containment structure. Six frac tanks installed within secondary containment. Erosion and Sedimentation Plan in mailbox. E/S best management practices installed and maintained including sediment trap. Vegetative growth well established in previously disturbed areas. No apparent E/S issues.

Six 500 bbl tanks installed within stainless steel containment structure. Six frac tanks installed within secondary containment. While onsite meet with Nate Smiell - Range Resources for the purpose of conducting a Notice of Termination inspection. PCSM BMPs installed and maintained. Permanent Stabilization achieved according to PA Code 25 Chapter 102.22(a)(2).

The Notice of Termination request is approved.

2723483 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted On Tuesday, April 17, 2018, I conducted a routine-complete inspection at the Mocek John 11222 Well Site located in Smith Township, Washington County.

Gas well 4H and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was labelled.

No violations were identified during the inspection.

2758241 Primary Facility 2018-07-30 Routine/Complete Inspection No Violations Noted On 7/302018 at approximately 1020, I conducted a routine site inspection of the Mocek John Well Site located in Smith Township, Washington County to assess the site's overall restoration and stability. The following was observed:
  • Act 9 sign at the access road entrance was in good condition.
  • Wells on site were tagged and appeared to be in good condition.
  • Production fluid/condensate tanks were in secondary containment and in good condition.
  • GPUs were in good condition.
  • Retention pond to the south of the well pad was stable and appeared to be functioning properly.
  • The site had greater than 70% uniform vegetation coverage and appeared to be stable.

No violations were found during this inspection.
