The well apparatus and brine tank appear to be in sound condition. No observation of accelerated erosion or sedimentation in relation to the well pad area or access road. The site appears to be restored.
2295068 Primary Facility 2014-08-13 Incident- Response to Accident or Event Violation(s) Noted Inspection the result of a spill reported by the Operator on 8/12/2014 at 16:00. The Operator reported a brine spill of approximately 30 gallons. At the time of inspection the department observed two brine tanks manifolded together via a 2" steel pipe inside an earthen dike. The pipe was disconnected which appears to be the cause of the spill. Also observed was dead vegetation inside the earthen dike surrounding the brine tanks. No waterways appear to be impacted at this time. The Department suggests the Operator take samples of the contents of the brine tanks located onsite, the impacted soil before and after excavation and a background sample to ensure the affected material has been removed.
2306197 Primary Facility 2014-09-24 Compliance Evaluation Outstanding Violations - No Viols Req'd Compliance Inspection the result of a spill reported by the Operator on 8/12/2014 at 16:00. The Operator reported a brine spill of approximately 30 gallons. At the time of inspection the Operator has reconnected the manifold, excavated the affected material, and reseeded the area inside the earthen dike. The Department has not received the confirmatory samples requested in inspection # 2295068 dated 8/13/2014.

The excavated areal has been backfilled with clean material and it appears that the Operator has re-seeded are area. The manifold connecting the two brine tanks together has been reconnected.

2551522 Primary Facility 2017-01-03 Incident- Response to Accident or Event In Compliance with Policy Inspection performed in response to Operator’s report of a 7bbl brine spill caused by a broken valve (CTS ID 324358). Operator stated that on 12/30/16 @ 12:00, well tender noticed the discharge valve on one of the two brine tanks had frozen and cracked, resulting in the release. Release was contained to earthen dike and was evacuated on the same day and hauled for disposal. Operator estimated the release volume based on the most recent tank readings.

At the time of inspection, the two brine tanks within the earthen dike had been disconnected from production equipment and temporarily moved off site and placed on containment pending ongoing clean-up. Department did not observe any standing liquid within the earthen dike. Department did not observe any signs (indicated by visibly stained vegetation and/or soil) of spill breaching the dike walls.

Department recommends Operator perform clean-up in accordance with spill policy and forward copies of all sampling test results upon receipt. Department will conduct
