1747698 Primary Facility 2008-10-16 Routine/Complete Inspection No Violations Noted Found the following deficiencies with the submitted E&S plan: 1) More detail is required for the 2nd culvert installation over the unamed tributary. 2) Specific BMP's should be shown and implemented at the culvert inlets & outlets on the E&S plan drawing. In addition culverts are ineffective as BMP's during well development and should be used in final restoration. 3) Additional BMP's should be shown & installed on the access road approaching the well-site. 4) The Department reccomends that the operator rock the access approaching the culverted stream crossing on either side to insure the stability of the road and the minimization of erosion & sedimentation. 5) Due to the slope & proximity of the site to the UNT the filter fabric fence should be backed with hay bales.
