At the time of inspection the site was being constructed. The E&S plan was on site and was properly implemented. The site was temporary stabilized as outlined in the E&S Plan. The Department notes an unlined pit onsite at the time of inspection. No accelerated erosion observed in relation to the access road or well pad area at the time of the inspection. 2184279 Primary Facility 2012-07-23 Incident- Response to Accident or Event Violation(s) Noted Inspection in response to an inadvertent return reported of approximately 140 barrels 80 barrels of fresh water and 40 barrels of betonite, 20 barrels of type one cement on Saturday July 21 2012 at 10:30pm. The Operator was in compliance with the spill policy when they reported to the Department during non-business hours. The Departments emergency response personnel John Carson conducted an inspection on Sunday July 22. Mr. Carson documented the result and supplied the Water Quantity Specialist with photographs and description of the discharge. Monday afternoon I conducted a follow-up inspection of the affected area. The Department notes that the Operator has plugged the culvert pipe and stationed a pump truck to cut off any runoff from entering the steam and reservoir directly down gradient from the discharge spring. 2184280 Primary Facility 2012-07-30 Compliance Evaluation No Violations Noted Compliance inspection in response to IR reported on 7/21/2012. After reviewing the sample results and comparing them to background samples, the stream affected by the IR has been restored. The Department notes that the pump truck has been removed and the stream naturally flushed by recent significant precipitation events. At the time of inspecting the Department notes that the site appeared to be in sound condition with no sign of accelerated erosion and sedimentation in relation to the access road or well pad area.
