The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2497578 Primary Facility 2016-06-28 Complaint Inspection No Violations Noted At the time of inspection the well was drilled and equipped for production. The well apparatus, brine tanks, and gas production units appeared to be in sound condition.

Inspection also in response to complaint 320656, the complaint indicates that “waste water from the site contains elevated LEL levels and routinely flashes and is being mixed with other wastes”. During the onsite inspection the brine tank appeared to be sound condition and no observation of onsite mixing was observed by the Department. This complaint is being forward to Waste Management for follow up at the waste disposal facility listed in the complaint.

The site ID was installed. The PPC plan and Emergency Response plan were onsite. The wells appeared to be drilled and equipped for production. The Department notes that the Operator was actively using the well pad site for parking associated with the earth disturbance activity of an adjacent pipeline. According to paperwork on site the well site earth disturbance permit was issued by the Westmoreland County Conservation District. The photo attached refers to WCCD Permit #8538. The post construction storm water management mapping indicates that an infiltration basin and two infiltration berms were to be installed. The site appeared to be constructed in accordance with the PCSM mapping, however the infiltration berms were holding water and the infiltration basin was empty. The Department observed a cone shaped stain under the 6 inch white plastic emergency overflow inside the concrete outlet structure associated with the infiltration basin. The Department also observ

2617150 Primary Facility 2017-07-20 Follow-up Inspection No Violations Noted Follow up inspection to complaint 326480. At the time of inspection the well was drilled and equipped for production. No observation of accelerated erosion or sedimentation. The PCSM retention basin appeared to be functioning. The Department observed recent earth disturbance in the area up gradient from the retention basin near the PCSM retention berms. According to the inspection conducted by the Westmoreland County Conservation District on 5/26/2017 the Conservation District recommended the Operator install under drains. The area of earth disturbance suggests the recommendation has been followed. The Department recommends the Operator continue to monitor the disturbed area to prevent accelerated erosion and sedimentation and ensure the under drains function properly.
