| I inspected this salt water injection well as a general, routine status check. The facility is comprised of an aboveground storage tank (AST) battery with 9 210-barrel steel tanks in a common containment pad. Adjacent to the tank impoundment, is a combined maintenance/office/control room. The wellhead is located 320 feet behind (southwest) of the control building. Unloading is executed by attaching 4-inch flex-hose to cam-lock fittings at the pit area and fluids are drained into this open-topped, cement-sided impoundment structure which is underlain by a series of pre-cast cement tank vaults. Fluids are moved from the pit into the battery tanks via fluid pumps with flexible hoses. Secondary containment at the battery is comprised of both cement walls and berms. The berms consist of gravel-ballasted flexible liner over coarse-granular or gravel dike.  Areas surrounding the AST impoundment and control building consist of parking and drive areas which are gravel and/or cinder-paved. The areas beyond the footprint of the facility are comprised of woods and low-lying marshy and bog-type terrain. I inspected the low-lying marshy and bog areas surrounding the facility and did not observe any sheens, free-oil, stressed vegetation, or other signs of leakage or spills. I observed the drainage culvert along the roadway at the entrance to be similarly free from staining, sheens, stressed vegetation, or other signs of impact. The well is located in a locked, board-on-board enclosure in a small clearing behind the facility. I observed connections and fittings at the wellhead to be secure and free from signs of escaping gases, fluids, or signs of leakage. At the time of my inspection, fluid was being off-loaded from a tank truck into the pit. At the time of my inspection, the interior of the impoundment was dry. I observed the horizontal and vertical surface areas of the dike structures to be free from staining. Exterior surface areas beyond the containment walls and berms was free from signs of active or prior leakage.

Additional permit requirements may be warranted.
+
| An inspection found the well exceeding the maximum allowable injection pressure.  The well tender was instructed to slow the pump rate, lowering the pressure by more than 20 psi. on the tubing.  The well owner was instructed to put a minimum pressure of 200 psi. on the tubing/casing annulus.

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1018249880 2014-10-30 The injection well and surface facility were inspected. There were no violations found at the time. Field Visit or Inspection
1023157577 2013-09-26 No action has been taken to convert the well from production to injection.
-1171463952 2013-11-22 The well was flowing gas and water. The owner was required to postpone packer installation.
-1236373346 2013-11-26 A mechanical integrity test was performed on the well. 1135 psi. of pressure was placed on the tubing/casing annulus. The test passed.
1349118946 2012-09-11 The well owner has applied to convert the well from production to saltwater injection. The well site was inspected as part of a field review. There are no water wells within 300 feet of the well and proposed facility location. The well is located within what may be wetlands. Additional permit requirements may be warranted.
1384800522 2013-11-15 The 4-1/2 inch casing was tested with 1180 psi. of pressure. The pressure held, without a loss, during a fifteen minute test period.
1412517557 2014-08-27 A sample was taken from the facility. The sample, number SJO-117, was taken to the lab for analysis.
1428508885 2015-04-08 I inspected this salt water injection well as a general, routine status check.
-1502281984 2014-07-25 An inspection found the well exceeding the maximum allowable injection pressure. The well tender was instructed to slow the pump rate, lowering the pressure by more than 20 psi. on the tubing. The well owner was instructed to put a minimum pressure of 200 psi. on the tubing/casing annulus.
1585393960 2014-09-11 No violations were found at the time.
1616035129 2013-11-19 The bottom hole cement plug was tagged at 4169 feet. A bridge plug was set at 4160 feet. The well was treated with acid. Conditions were acceptable.
-1745147343 2013-11-25 The packer installation was witnessed. A Weatherford dual element packer was ran on 2-7/8 inch tubing.
1785123789 2014-01-31 There were no violations found at the time.
-196934763 2015-01-12 The injection well and surface facility were inspected. There were no violations found at the time.
-283340957 2015-02-13 The injection well and surface facility were inspected. The annulus was frozen. There were no violations found at the time.
-472998540 2013-11-12 Contractor will blow down the well and attempt to tighten 4 1/2 inch casing.
-649082053 2014-06-10 The pump was operating at the time. No violations were found.
671613497 2014-05-16 The pump was not operating. No violations were found at the time.
716047444 2013-12-04 A mechanical integrity test was performed on the flow line. A pressure of 1080 psi. was held on the flow line for fifteen minutes. The test passed.
772248291 2014-12-04 The injection well and surface facility were inspected. There were no violations found at the time.
869889491 2014-03-18 The well was not in operation at the time. There were no violations found.
-936833964 2015-03-19 The injection well and surface facility were inspected. The annular pressure was not gauged. There were no violations found at the time.
